心肌梗死患者溶栓前后的多项炎性因子的变化规律研究

摘　　要：目的:探讨心肌梗死患者溶栓前后的多项炎性因子及VEGF、sICAM-1、sVCAM-1、MMP的变化规律。方法:选取2009年7月～2012年3月于本院进行溶栓治疗的56例心肌梗死患者为观察组,同时期的56名健康人员为对照组,将观察组溶栓前、溶栓后1、3、5、7d和对照组的血清炎性因子和VEGF、sICAM-1、sVCAM-1、MMP-2、MMP-9水平进行检测及比较。结果:观察组术前血清炎性因子及VEGF、sICAM-1、sVCAM-1、MMP-2、MMP-9均高于对照组,而观察组溶栓前至溶栓后1、3、5、7d呈现升后降的趋势,而CRP于溶栓后3d、其他指标则于溶栓后5d至峰值,差异均有统计学意义(P<0.05)。结论:心肌梗死患者溶栓前后的血清炎性因子及VEGF、sICAM-1、sVCAM-1、MMP呈现先升后降的趋势,可作为监测溶栓效果的有效指标。Objective.. To study the changes in levels of multiple inflammatory factors and VEGF, slCAM-1, sVCAM-1, MMP in patients with miocardial infarction before and after undergoing thrombolytic therapy. Methods： A total of 56 patients with miocardia; infarction who were treated with thrombolytic therapy in our hospital during July 2009 and March 2012 were enrolled as observation group, and 56 health- y person were also selected as control group,then the serum levels of inflammatory factors and VEGF, sI- CAM-I,sVCAM-1,MMP of observation group before undergoing thrombolytic therapy and 1, 3, 5, and 7 days after the therapy were compared with that of the control group. Results .. Before undergoing thrombo- lyric therapy, levels of serum inliammatory lactors and VEGF, slCAM-1, sVCPkM-1, MMP-2, MMP-9 of observation group were all higher than those of the control group, but these levels showed an increasing and then decreasing tendency after undergoing the therapy. In the observation group, CRP level reached the peak 3 days after undergoing the therapy, but for other levels, the peak appeared 5 days atter the ther- apy showing significant difference （P（0.05）. Conclusion;The increasing and then decreasing tendency of levels of serum inflammatory factors and VEGF, sICAM-1, sVCAM-1, MMP before and after undergoing thrombolytic therapy in patients with miocardial infarction can be used as indicators for judging the treat- ment effects of the therapy.
