不同针灸方法治疗带状疱疹时程效应与止痛作用的关系

摘　　要：目的观察不同针灸方法治疗带状疱疹急性期时程效应与止痛作用的关系。方法采用多中心、随机对照的方法,将符合要求的94例带状疱疹急性期患者随机分为4组,A组(27例)予电针治疗,B组(23例)予电针+铺棉灸治疗,C组(24例)予电针+火针治疗,D组(20例)予电针+叩刺拔罐治疗。均每日1次,10 d为1个疗程。观察其时程效应与止痛作用的关系。结果 4种针灸方法治疗带状疱疹的总体止痛作用相当(P>0.05);治疗1、2、3、8、10 d,4组患者的疼痛强度存在差异(P<0.05);治疗2 d,电针+铺棉灸组的止痛作用较其他3组弱(P<0.05);治疗4～7 d,电针组的止痛作用优于电针+铺棉灸组(P<0.05)。结论本研究的4种针灸方法治疗带状疱疹的总体止痛作用相当,在治疗2 d、4～7 d不推荐采用电针+铺棉灸法用于疼痛治疗。Objective To observe the relationship between time-distance effect and analgesic effect in the treatment of acute herpes zoster by different acupuncture methods.Methods A multiple-center,randomized and controlled trial was carried out in 94 herpes zoster patients at acute phase.The patients were randomized into 4 groups：A group with 27 patients was given electroacupuncture therapy,B group with 23 patients was given electroacupuncture combined with cotton-pave moxibustion,C group with 24 patients was given electroacupuncture combined with fire acupuncture,D group with 20 patients was given electroacupuncture combined with pricking-cupping bloodletting therapy.All patients received the treatment once per day for one course of 10 days.The relation between the time-distance effect and analgesic effect was observed.Results The analgesic effects by four different acupuncture methods were almost the same（P0.05）.At the 1st,2nd,3rd,8th,10th day of the treatment,there were differences in the pain degree among the four groups.At the 2nd day of the treatment,the analgesic effect of electroacupuncture combined with cotton-pave moxibustion group was less than the other three groups（P0.05）.The analgesic effect of electroacupuncture group was better than electroacupuncture combined with cotton-pave moxibustion group during the 4th-7th day of the treatment（P0.05）.Conclusion The analgesic effects of the four different acupuncture methods are almost the same.The electroacupuncture combined with cotton-pave moxibustion therapy is not recommended at the 2nd day and the 4th-7th day of the treatment to relieve the pain.
