摘  要:目的:探讨益心解毒方对心梗后心衰大鼠损伤心肌的保护作用机制。方法:利用左冠状动脉结扎术制备大鼠心梗后心衰动物模型,通过宏观表征结合理化指标综合判定该疾病模型的证候属性,并给予对证中药的干预。检测各组大鼠的心脏超声指标;通过血生化的方法检测血清中肌酸激酶(CK)的水平以及利用酶联免疫的方法测定血清中肌钙蛋白I(CTNI)的水平;并通过放免的方法检测血浆中内皮素(ET)与降钙素基因相关肽(CGRP)的浓度。结果:术后21d,模型组大鼠的心功能显著下降,主要表现在左室舒张末内径(LVEDd)、左室收缩末内径(LVEDs)明显增大,短轴缩短率(FS)以及射血分数(EF)急剧下降(P<0.01);血清中CK、CTNI水平显著升高(P<0.01);血浆中ET水平升高(P<0.01),CGRP水平下降(P<0.01)。给予益心解毒方干预后,能够有效的降低心衰大鼠扩张的LVEDd和LVEDs(分别下降19%和25%,P<0.05和P<0.01),尤以改善LVEDs为主(P<0.01),提高EF(提高25%,P<0.01);并且降低心衰大鼠血浆中上升的ET浓度(P<0.05),升高血浆中CGRP浓度(P<0.01),且进一步降低ET/CGRP的比值(P<0.01);显著下调心衰大鼠的血清中CK、CTNI(P<0.01,P<0.05)。结论:益心解毒方能够有效的提高心衰大鼠的心功能,其机制与降低血中ET、CK、CTNI,并升高血中的CGRP水平等心肌保护作用有关。Objective:To clarify myocardial protection and possible mechanism of Yixin Jiedu Formula(YXJDF) on the model of heart failure induced by post-infarction which was established as Qi deficiency and blood stasis syndrome.Methods: Model of heart failure after myocardial infarction was prepared by left coronary artery ligation.Syndrome were evaluated by combining macroscopic symptoms with biochemical indicators.Drugs were delivered by continuous intragastric administration for 21d.Then cardiac function of each group was evaluated by Echocardiography.The level of creatine kinase(CK) in serum were detected by blood biochemistry and the level of cardiac troponin I(CTNI) were detected by immunoturbidimetric assay.Plasma endothelin(ET) and calcitonin gene-related peptide(CGRP) levels were also examined by radioimmunoassay(RIA) in all groups.Results:Compared with normal group,the left ventricular end-diastolic diameter(LVEDd) and left ventricular endsystolic diameter(LVEDs) of model group increased significantly,ejection fraction(EF) and fractional shortening(FS) decreased significantly(P0.01).CK and CTNI in serum showed a high level in model group(P0.01).RIA results showed that ET of model group in plasma was significantly higher(P0.01) than normal group and CGRP was lower(P0.01).The YXJDF could improve EF and FS,further to improve the cardiac function.YXJDF could down-regulate the plasma ET and up-regulate the plasma CGRP(P0.05) respectively.And the experimental results showed that YXJDF could reduce high levels of CK and CTNI in serum and show a protective effect on heart.Conclusion: YXJDF could strength the cardiac function and the mechanism might be that it could decrease plasma ET levels and reduce CK,CTNI in serum,but improve the release of plasma CGRP.
