摘  要:目的探讨α-平滑肌肌动蛋白(α-SMA)在增生性玻璃体视网膜病变(proliferative vitreoretinopathy,PVR)增生膜中的表达以及血小板源性生长因子(PDGF)对人视网膜色素上皮(RPE)细胞表达α-SMA的影响。方法通过免疫荧光实验和免疫组化法对14例PVR患者视网膜表面增生膜(PRM)中α-SMA的表达进行定性及定量分析;用外源性PDGF-BB处理体外培养的人RPE细胞,并通过免疫荧光实验检测PDGF-BB对RPE细胞表达α-SMA的影响。结果免疫组织化学结果显示:α-SMA在14例不同级别的PRM中均有表达,主要分布在胞浆中,但其表达的程度有差异。α-SMA阳性细胞在PVR/C级PRM膜中比率为35/80,在PVR/D级PRM膜中的比率为50/60。免疫荧光定量分析结果显示:α-SMA在C级PRM膜和D级PRM膜中的平均荧光强度分别为12.31和23.09,二者差异有统计学意义(P<0.01)。外源性的PDGF-BB(50μg.L-1)能显著促进人RPE细胞中α-SMA的表达(50μg.L-1PDGF-BB处理前后平均荧光强度分别为10.08和17.23),差异有统计学意义(P<0.05),这种促进作用在培养基中有血清存在时明显增强。结论α-SMA在PVR增生膜中广泛表达,其表达量与PVR病变程度有关,提示α-SMA可作为检测PVR病变程度的一个潜在因子;PDGF促进α-SMA的表达,这提示PDGF在PVR发病中有重要作用,为临床上PVR的预防和治疗提出新的思路。Objective To investigate the expression of a-smooth muscle actin (α-SMA) in periretinal membrane (PRM) of proliferative vitreoretinopathy (PVR) and the effects of platelet derived growth factor(PDGF) on the expression of α-SMA in cultured human retinal pigment epithelial (RPE) cells. Methods The expression of α-SMA in 14 specimens of PRM was qualitatively and quantitatively analyzed by immunohistochemistry and immunofluorescence. Exogenous PDGF-BB was administrated to human RPE cells and the effects of PDGF-BB on the expression of α-SMA by human RPE cells was determined by cell climbing slices assay and immunofluorescence. Results Immunohistochemistry showed that α-SMA expressions were observed in 14 PRM, which was mainly distributed in cytoplasm, but the expression degree had differences. α-SMA positive cells in C grade of PRM and D grade of PRM accounted for 35/80 and 50/60 ,respectively,while the average fluorescence intensity were 12.31 and 23.09, respectively, there was significant difference ( P 〈 0.01 ). Exogenous PDGF ( 50 μg·L^-1) could promote the α-SMA expression in human RPE cells, and the average fluorescence intensity of α-SMA before and after PDGF-BB(50μg·L^-1 ) treatment were 10.08 and 17.23, there was statistical difference (P 〈 0.05 ), the promotive effect was obviously increased when the cultured medium contained blood serum. Conelusion α-SMA was extensively expressed in PRM, and its expression was related with the pathological grade of PVR, which indicates that α-SMA can be as a potential marker for the detection of PVR. PDGF can promote the expression of α-SMA, which suggests that PDGF play vital roles in PVR. Further research aimed at the exact role of α-SMA and PDGF may provide new therapeutics for PVR.
