自制网袋捆绑破裂肾微创与开放手术的疗效比较及护理评估

摘　　要：目的评估手助腹腔镜改良自制网袋肾捆绑术与开放手术治疗肾破裂的疗效与护理。方法收集我院30例肾破裂患者资料,采用改良自制网袋肾捆绑术,共分二组,组1:患者16例,选择开放手术。组2:患者14例,选择经腹腔手助腹腔镜途径,比较两组各项指标,评估其临床疗效与护理措施。结果手助腹腔镜组手术完成率100%。微创组和开放组的肾切除率分别为1/14和1/16,两组在平均出血量、平均饮食恢复时间、术后平均住院时间、1个月内体重平均增加等方面有显著差异。结论自制丝线网袋肾捆绑术治疗肾破裂手术疗效确切,而微创手术具有损伤小、出血量少、恢复快、体力恢复良好的优势,加强术前术后护理对促进肾破裂术后恢复作用明显。Objective To compare the clinical effects and nursing care of binding kidney with home-made string bag un- der hand-assisted laparuscopy for renal rapture or open surgery. Methods Colleced 30 cases of patients with renal rupture. Using binding kidney with home-made string bag. They were divided into two groups,Group 1： 16 patients,by retroperi- toneal open channels. Group 2：14 patients, using hand-assisted laparoscopy by abdominal cavity channels.Compared with the indicators of the two groups, and assessed their clinical efficacies. Results Completion rate of the surgery of hand-as- sisted laparoscope group was 100%. Nephrectomy rates of Group 2 and Group 1 were 1/14 and 1/16.In the average amount of bleeding, the average time to resume diet ,the average length of hospitalization after operation and the average increasing of body weight within one month ,there were significant differences. Conclusion Binding kidney with home-made string bag is feasible and effective. 14and-assisted laparoscopic has the advantages of less bleeding and restore faster physical recov- cry.
