塞来昔布对全膝关节置换围手术期镇痛的疗效评估

摘　　要：目的观察塞来昔布(西乐葆)对全膝关节置换围手术期患者术后镇痛的疗效和安全性。方法行单侧人工全膝关节置换的患者50例,随机分为两组,实验组术前术后均口服塞来昔布,对照组仅术后使用静脉用自控镇痛泵,观察人工全膝关节置换后患者疼痛程度和膝关节的活动度。结果实验组术后6～48 h静息疼痛VAS评分显著低于对照组。两组术后关节功能方面显示,实验组术后2 d、14 d被动活动度显著高于对照组;实验组和对照组术后恶心、呕吐发生率分别为40%和44%,差异无统计学意义(P>0.05);术中出血量研究组为(181.9±5.4)mL,对照组为(177.4±6.9)mL;术后出血量研究组为(544.7±2.7)mL,对照组为(511.4±3.1)mL;术后输血量研究组为(385.5±7.3)mL,对照组为(393.3±6.7)mL,上述指标两组对比结果均无统计学意义(P>0.05)。结论西乐葆对手术后镇痛具有满意的疗效和安全性,能明显改善术后2周内的关节被动活动范围,适合于人工全膝关节置换围手术期术后镇痛。Objective To observe the efficacy and safety of celecoxib （Celebrex） on total knee replacement in the operation period of patients with postoperative analgesia. Methods Unilateral total knee replacement in 50 patients,were randomly divided into two groups. The experimental group：before and after operation were treated with oral celecoxib; the control group： only postoperative intravenous patient-controlled analgesia pump. Observed on artificial total knee arthroplasty in patients with knee pain severity and activity. Results The experimental group after 6-48 h VAS resting pain score was sig- nificantly lower than that of control group. Two groups of joint function after operation,patients of the experimental group 2,14 d passive activity was significantly higher than that in control group.Postoperative nausea and vomiting incidence rate,the experimental group was 40% and the control group was 44%. the project two group showed no statistical difference ISO（P 〉 0.05）. The amount of bleeding in experimental group was （181.9＋5.4） mL, （177.4_＋6.9） mL in the control group; postoperative bleeding study group was （544.7_＋2.7） mL, （511.4_＋3.1 ） mL in the control group; postoperative blood transfusion volume study group was （385.5_＋7.3） mL, （393.3_＋6.7） mL in the control group, the project two groups showed no statistical difference ISO（P 〉 0.05 ）. Conclusion Celebrex on analgesia after operation with satisfactory efficacy and safety,can significantly improve within 2 weeks after the operation of passive joint motion range, suitable for artificial total knee re- placement in the operation period of postoperative analgesia.
