新生儿抚触法在早产儿喂养不耐受治疗中的临床效果分析

摘　　要：目的比较新生儿抚触法结合非营养性吸吮与传统药物方法治疗早产儿喂养不耐受的临床效果。方法选取河北北方学院附属第三医院2008年1月～2011年1月的喂养不耐受早产儿80例,随机分为药物治疗组(40例)、新生儿抚触结合非营养性吸吮组(简称抚触与NNS治疗组)(40例)。两组均在常规治疗基础上,药物治疗组给予红霉素治疗喂养不耐受,抚触与NNS组实施抚触和非营养性吸吮。观察两组治疗效果。结果药物治疗组患儿的呕吐时间[(1.8±0.6)d]、胃残余量[(2.7±1.1)mL]、体重增加[(337.0±35.9)g]、奶量增加量[(2.3±0.5)mL]、治疗总有效率(65.0%),和抚触与NNS治疗组患儿的呕吐时间(1.7±0.7)d、胃残余量[(2.4±1.5)mL]、体重增加情况[(336.0±37.3)g]、奶量增加量[(2.4±0.6)mL]、治疗总有效率(62.5%),两组比较,差异均无统计学意义(均P>0.05)。结论非营养性吸吮配合新生儿抚触不但有较好的治疗效果,而且没有不良反应,同时又能促进早产儿自身消化系统的发育,值得临床推广。Objective To compare clinical effects of traditional medicines and infant massage combined with the non-nutritive sucking to treat preterm children feeding intolerance.Methods From January 2008 to January 2011 in the Third Affiliated Hospital of Hebei North University,feeding intolerance in preterm infants of 80 cases,40 cases were randomly divided into medicines treatment group,infant massage combined with 40 cases of non-nutritive sucking（massage and NNS group）.Both groups were given conventional therapy,on the basis of conventional therapy,the medication treatment group were treated with Erythromycin in the treatment of feeding intolerance,massage and NNS treatment group was the implementation of touching and non-nutritive sucking.Results The drug treatment group′s vomiting [（1.8±0.6）d],gastric residual volume [（2.7±1.1）mL],weight gain [（337.0±35.9）g],and milk increase [（2.3±0.5）mL],total effective rate was 65.0%,and massage and NNS treatment group′s vomiting time [（1.7±0.7）d],gastric residual volume [（2.4±1.5）mL],weight gain [（336.0±37.3）g],and milk increase [（2.4±0.6）mL],treatment and the total efficiency was 62.5%,two groups of circumstances,There was no significant difference between two groups（all P 0.05）.Conclusion Non-nutritive sucking with the infant massage is not only better treatment,but also no adverse reactions,while promoting the development of the digestive system in preterm infants,it is worthy of promotion.
