椎板支撑钢板对颈椎单开门轴性症状的影响

摘　　要：目的探讨在颈椎单开门椎管扩大成形术(ELAP)中应用椎板支撑钢板固定技术对术后轴性症状的影响。方法选择我院2007年6月～2011年2月采用传统缝线悬吊ELAP治疗的31例脊髓型颈椎病患者为A组,同期采用传统缝线悬吊辅以椎板支撑钢板固定ELAP治疗的35例患者为B组,比较术后及术后1年两组间JOA评分改善率、轴性症状发生率。结果术后及术后1年两组间JOA评分改善率[A组为(58.57±2.32)%和(61.09±5.14)%,B组为(59.83±3.16)%和(62.51±4.13)%]比较,差异无统计学意义(P>0.05);术后轴性症状发生率A组为41.94%(13例),B组为17.14%(6例),差异有统计学意义(P<0.05);术后1年轴性症状发生率A组为3.23%(1例),B组为2.86%(1例),差异无统计学意义(P>0.05)。结论传统ELAP中应用椎板支撑钢板固定技术能减少术后近期轴性症状的发生,但对术后远期的轴性症状无影响。Objective To explore the effect of ELAP with laminae bracing plate fixation in the treatment of cervical spondylotic myelopathy on the postoperative axial symptoms.Methods 31 cases with cervical spondylotic myelopathy from January 2007 to February 2011 in our hospital were selected to receive ELAP by traditional suture suspension（group A）,35 cases were treated with ELAP by traditional suture suspension and laminae bracing plate fixation（group B）.The postoperative and 1 year after the operations neurological status were evaluated by JOA scoring system and the improvement rate was also calculated;surgical complications such as postoperative axial symptoms were also recorded.Results The difference of improvement rate of JOA after operation and 1 year later between group A [（58.57±2.32）% and（61.09±5.14）%] and group B [（59.83±3.16）% and（62.51±4.13）%] was not statistically significant（P 0.05）.The difference of incidence rate of postoperative axial symptoms between group A（41.94%,13 cases） and group B（17.14%,6 cases） after operation was statistically significant（P 0.05）,and The difference of incidence rate of axial symptoms 1 year later between group A（3.23%,1 case） and group B（2.86%,1 case） was not statistically significant（P 0.05）.Conclusion ELAP with laminae bracing plate fixation can reduce recent postoperative axial symptoms but cannot cause long-term effects on the axial symptoms.
