摘 要:背景:骨髓间充质干细胞具有自我增殖能力和分化多潜能性,β-磷酸三钙陶瓷人工骨具有良好的细胞亲和力及良好的生物相容性,是治疗股骨头坏死的热点。目的:观察骨髓间充质干细胞联合β-磷酸三钙对兔股骨头坏死的修复作用。方法:抽取兔自体骨髓并分离和培养骨髓间充质干细胞,扩增至3代后与β-磷酸三钙复合;24只兔建立双侧股骨头坏死动物模型,48侧股骨头随机分为3组,空白组制作缺损而不填充任何材料;β-磷酸三钙组单纯填充β-磷酸三钙;复合组填充β-磷酸三钙和骨髓间充质干细胞的复合材料。结果与结论:修复后8周复合组成骨细胞较多,新生骨小梁相互连接成片;β-磷酸三钙组骨量少,部分纤维组织填充;空白组缺损区靠近宿主骨区域可见少许软骨细胞及软骨组织,纤维组织较多。修复后4,8周复合组骨缺损平均阻射密度较β-磷酸三钙组、空白组增高(P<0.05);β-磷酸三钙组与空白组相比差异无显著性意义(P>0.05)。采用Lane-Sandhu法评分标准,4,8周复合组骨小梁面积均高于β-磷酸三钙组及空白组(P<0.05)。提示骨髓间充质干细胞有较强的成骨作用,有助于股骨头坏死缺损的修复。BACKGROUND:Bone marrow mesenchymal stem cells have self-proliferative capacity and pluripotent differentiation.β-tricalcium phosphate (β-TCP) ceramics has good cell affinity and biocompatibility.It is a hot spot for avascular necrosis of the femoral head.OBJECTIVE:To investigate the repair effect of bone marrow mesenchymal stem cells combined with β-TCP on avascular necrosis of the femoral head in rabbits.METHODS:First,bone marrow mesenchymal stem cells were isolated from autologous bone marrow in rabbits and cultured.Then,bone marrow mesenchymal stem cells were combined with β-TCP after amplification to the third generation.Totally 24 rabbits model of bilateral avascular necrosis of the femoral head was established.The 48 sides of femoral head were randomly divided into three groups:bone defects filled with no material as the blank group,and those filled with β-TCP as the β-TCP group,while those filled with β-TCP+bone marrow mesenchymal stem cells as the composite group.RESULTS AND CONCLUSION:Osteoblasts cells in the composite group were increased and new-formed trabecular were connected together at week 8 after repair.While a few new bone formed in the β-TCP group,and some connective tissues grew into the defects.There were a few chondrocytes and cartilage tissues as well as many fibrous tissues near the host bone area in the defect area of the blank group.Average resistance shot density in bone defects of the composite group was higher than that of β-TCP and blank groups at weeks 4 and 8 after repair (P 0.05).There was no significant difference between the β-TCP and blank groups (P 0.05).The results of Lane-Sandhu scores showed that trabecular bone areas in the composite group was higher than those in the β-TCP and blank groups at weeks 4 and 8 after repair (P 0.05).These results suggest that bone marrow mesenchymal stem cells have a strong activity of osteogenesis,contributing to repairing avascular necrosis of the femoral head.
