摘  要:目的探讨外源性肾上腺髓质素(ADM)对肾脏机械性损伤早期肿瘤坏死因子-β(TNF-β)及肿瘤坏死因子受体(TNFR)表达的影响。方法选健康成年普通级Wistar大鼠104只,随机分为四组:对照组8只、单纯创伤组32只、创伤前注射ADM组32只、创伤后注射ADM组32只,后三组的动物分别于创伤后1 h、6 h、12 h和24 h各处死8只。采用自由落体打击仪直接打击大鼠脊肋区制作肾机械性损伤模型。ADM分别于创伤前或创伤后10 min采用腹腔注射法给药。实验动物采用快速心脏采血法处死。迅速解剖动物提取肾脏标本,10%甲醛固定。免疫组化染色观察TNF-β、TNFR在组织中的表达。结果单纯创伤组于创伤早期(1 h和6 h点)TNF-β表达低于对照组,外源性ADM早期(1 h和6h点)可上调TNF-β(P<0.05)。单纯创伤组于创伤早期(1 h和6 h点)TNFR表达低于对照组,外源性ADM(创伤前给药)早期(1 h和6 h点)可上调TNFR(P<0.05)。结论 TNF-β在肾脏损伤中主要参与的是抗损伤过程。TNFR对TNF-β具有动态调节作用。ADM在早期肾损伤过程中可以上调TNF-β、TN-FR,并与其相互协调,共同发挥对损伤器官的保护作用。Objective To investigate the effects of TNF-β and the acceptor expression about mechani- cal renal trauma with extragenous adrenomedullin (ADM). Methods 104 healthy adult plain grade Wister rats randomly divided into four groups : 8 in the group of control,32 in the group of trauma,32 in the group injected ADM before trauma,32 in the group injected ADM post trauma. The 8 rats in the last three groups were excuted at 1 h,6 hs, 12 hs and 24 hs post trauma respectively. The experimental model of rat kidney with mechanical trauma were prepared by striking the area of rat skin reflecting by kidney with free dropping ferreous hammer. ADM administrated by intrapertoneal injection at 10 min before trauma or post trauma respectively. All rats were executed by drawing-out all the blood in their hearts. Renal tissue was investigated to study positive expression of TNF-β, TNFR after SABC stained. Results The TNF-β expression of trauma group is less than it of control group in the wound early time ( 1 h and 6 hs). The extragenous ADM increased the expression of TNF-β ( 1 h and 6 hs) ( P 〈 0.05 ). The TNFR expression of trauma group is less than it of control group in the wound early time (1 h and 6 hs). The extragenous ADM (ADM injected before trauma) increased the expression of TNFR (1 h and 6hs) (P 〈 0.05). Conclusion What the TNF-β participated in renal trauma mainly is the anti-dam- age process. The TNFR could regulate the TNF-β in dynamic balancing. ADM increased the expression of TNF- β,TNFR at the early phase of acute trauma and supplied kidney with protection.
