长效胰岛素类似物致皮肤过敏

摘　　要：1例59岁男性患者，糖尿病病史23年，期间一直口服格列喹酮片和阿卡波糖片控制血糖。患者因口服降糖药血糖控制不佳，开始每晚睡前皮下注射10u地特胰岛素注射液，使用2～3d后，发现在注射部位出现高于皮肤表面直径约2cm的红斑、硬结。使用2周后，将胰岛素品种更换为甘精胰岛素注射液，每晚睡前皮下注射10U，2～3d后，患者出现同样的过敏反应。遂将患者的治疗方案改为生物合成人胰岛素注射液（诺和灵R注射液）10U、6U、8U三餐前皮下注射和精蛋白生物合成人胰岛素注射液（诺和灵N注射液）6U睡前皮下注射，患者未见明显的过敏反应。One 59-year-old male patient with a history of diabetes for 23 years was consistently administered gliquidone tablets and acarbose tablets to control blood glucose. Due to the poor glycemic control with oral hypoglycemic agents, he used insulin detemir 10 U before bedtime. Erythema of two centimeter appeared at injection site after two or three days injection. Then, insulin detemir was changed to insulin glargine 10 U before bedtime two weeks later. Two or three days later, the same reaction appeared. Ten days later, the patient was administered insulin Novolin R 10 U, 6 U, 8 U before each meal and insulin Novolin N 6 U before bedtime. Significant allergic reaction was not found.
