急慢性肝病患者血清C_3氨基端肽和层粘素水平与肝脏病理学改变的对比研究

摘　　要：目的:探讨急慢性肝病患者肝组织损害与血清C_3氨基端肽(P(?)P)和层粘素(LN)水平的关系。方法:采用放射免疫法测定了110例急、慢性乙型肝炎患者的血清P(?)P和LN水平,并与肝组织病理损害程度进行对比分析。结果:急性组和慢性轻度组与正常对照组比较其血清P(?)P和LN水平无显著性差异(P>0.05),而慢性中度、重度和肝硬变组与正常对照组、急性组和慢性轻度组比较差异显著(P<0.05或0.01);并且,慢性肝病肝纤维化程度越重其血清P(?)P和LN水平也明显升高(P<0.05或0.01)。此外,血清P(?)P、LN与胆红素(TBil)水平呈正相关((P<0.05或0.05).与白蛋白(A)、白/球比值(A/G)和凝血酶原活动度(PTA)呈负相关(P<0.05或0.01),而与血清丙氨酸转移醇(ALT)和γ球蛋白(γ—G)不相关(P>0.05)。结论:测定血清P(?)P和LN水平对判断慢性肝痛肝组织的炎症坏死活动程度和肝纤维化程度有较大的参考价值。Aim: To evaluate the correlation of the serum levels of the T-terminal propeptide of type I procollagen (P I P) and larninine (LN)and Liver pathological changes in acute hepatitis B and chronic Liver diseases. Methods ; Liver biopsies were performed and the concentrations of P I P and LN in serum were meassured by radioimmunoassay in 110 patients with acute hepatitis B (ABH) .chronic hepatitis B (CBH) and Liver cirrhosis (LC). Results: There was a significant difference between the levels of P I P and LN in serum in normal group (20), ABH (16) and the milder of CBH (28) as compared to patients with the middler (34) and server (18) of CBH and LC (14) (P<0. 05 or 0. 01). The serum total bilirubin (TBil) .albumin (A).albumin/globulin(A/G) .and prothrombin activity (PTA) were all obviously associated with the levels of P I P and LN of CBH, while the serum alanine aminotransferase (ALT) and r-globulin (γ-G) levels were not related to the levels of P I P and LN, The histological staging was significantly associated with the levels of P I P and LN in CBH (P<0. 05). Conclusion: Measerements of P I P and LN may be noninvasive tests for detection of necroinferction activity and hepatic fibrosis of CHB and chronic liver diseases in the liver tissues
