双胎不一致性生长的妊娠结局分析

摘　　要：目的分析双胎不一致性生长的妊娠结局。方法回顾性分析活产分娩的双胎妊娠108例临床资料,以双胎胎儿体重差>20%为诊断标准,分为研究组及对照组,比较2组间孕妇一般情况、妊娠期并发症、新生儿出生体重、围产儿死亡率、新生儿窒息率及入住NICU情况等方面的差异。结果研究组妊娠期高血压疾病的发生率为27.27%,高于对照组的20.93%(P<0.05)。研究组剖宫产分娩19例,占86.36%;对照组剖宫产分娩80例,占对照组总数的93.02%,2组比较差异无统计学意义(P>0.05)。研究组分娩胎儿44例,其中小于胎龄儿20例,占45.45%;新生儿平均出生体重、围产儿死亡率新生儿入住NICU比率明显高于对照组,差异有统计学意义(P<0.05)。结论不一致性双胎妊娠围生儿患病率和死亡率明显高于发育一致性双胎,及早发现和处理双胎发育不一致,是改善胎儿和新生儿预后的关键。Objective To analyze the perinatal outcomes of growth discordant in twins pregnancy.Methods A retrospective study was carried out on 108 cases of twin babies delivered and Jun.2012 and the cases were divided groups： study group and control group.Twin pairs whose birth weight difference was 20% or more were designated as discordant twins.To compare the difference between of the pairs about： Pregnant women in general,pregnancy complications,neonatal birth weight,perinatal mortality,neonatal asphyxia rate and occupancy rate of NICU.Results Gestational hypertension disease incidence was 27.27% in study group which was higher than that（20.93%） in control group,（P0.05）.Cesarean section delivery was 19 cases and 80cases in study group and control group,accounted 86.36% and 93.02%,there was no significant difference in the two groups（P0.05）.There were 20 cases less than gestational age in study group（45.45%）.Neonatal average birth weight,perinatal infants rate,neonatal occupancy rate in NICU was obviously higher in study group than that in the control group,and there was significant difference（P0.05）.Conclusion Inconsistency of twin pregnancy of perinatal morbidity and mortality rates are significantly higher in developmental consistency twin.Early detection and treatment of twin developmental inconsistency,is to improve fetal and neonatal prognosis.
