摘　　要：目的:探讨扩张Ⅰ期术后感染并发症的治疗、预防及致病原因。方法:对2010年4月～2012年5月扩张Ⅰ期术后发生感染并发症病例进行回顾性研究。结果:共22人28枚扩张器置入术后感染的病例中,通过将感染严重程度分级,针对轻、中、重度感染,分别采取全身应用广谱抗生素,扩张皮瓣表面外敷蒲公英等非手术治疗;放置引流管持续滴注引流;及时行扩张Ⅱ期手术等个性化治疗方法,19人25枚轻、中度扩张器感染均得到有效控制,Ⅱ期手术顺利,效果满意。3人3枚重度感染病例,及时进行扩张Ⅱ期手术,虽疗效欠佳,但结果仍可接受,避免更大损失。结论:扩张器感染并发症发生的原因较多,重在预防,发生后需及时处理,否则后果严重。通过密切观察,结合非手术方法,采取针对性治疗,感染并发症能得到有效控制,获得满意疗效。Objective To investigate the treatment,prevention and pathogenesis of the postoperative infective complications. Methods Retrospective study with the cases between April,2010 to May,2012 which had postoperative infection. Results In 22 patients who had 28 expanders＂ implantation totally that had postoperative infection,and graded by severity of infection,for mild, moderate and severe infection,respectively,to take a systemic broad-spectrum antibiotics,and wet compress the surface of the expansion flap with dandelion and use other non-surgical treatment,to place the drainage tube continuous infusion drainage, do some personalized treatment such as the expansion surgery at second stage timely,19 patients with 25 expanders who had mild to moderate dilator infection were controlled effectively, and the results of the surgery at second stage were satisfactory. 3 patients with 3 expanders had severe infections,do the expansion surgery at second stage timely, the results are poorly effective, but still acceptable, and to avoid greater losses. Conclusion There are much more reasons about the infection of expansion. Pay more attention on prevention, and deal with the problem after the occurrence immediately to avoid serious consequences.Through having observation frequently, combined with non-surgical methods, and taking targeted therapy,infectious complications can be effectively controlled, to obtain satisfactory results.
