摘  要:目的观察乌司他丁对重症有机磷中毒患者胆碱酯酶的影响,探讨其对有机磷中毒治疗的作用。方法重症有机磷中毒患者60例,随机分为对照组(n=30)和乌司他丁治疗组(n=30),观察胆碱酯酶的动态变化;记录胆碱酯酶活性恢复至80%的时间。同时观察阿托品化的时间、平均住院时间、脏器损害发生率等指标。结果入院后第7天,治疗组胆碱酯酶活力明显高于对照组,对照组胆碱酯酶恢复至80%的时间为(8.07±1.14)d,显著长于治疗组(6.77±0.97)d,P<0.05。两组阿托品化的时间无明显差别;治疗组并发症发生事件率16.7%低于对照组43.3%(P<0.05),住院时间也短于对照组。结论乌司他丁对重症有机磷中毒患者后期胆碱酯酶的恢复呈现有益的影响,并且可以保护脏器功能,提高对重症有机磷中毒的疗效。Objective To observe the effect of ulinastitin on cholinesterase in patients with severe organophosphorus poisoning.Methods Sixty patients with severe organophosphorus pesticide poisoning were randomly divided into control group(n=30) and ulinastatin treatment group(n=30).The dynamic changes of AChE were observed and the time was recorded.At the same time,the time of atropinization,the average length of stay,incidence of organ damage and other indicators were observed.Results On the seventh day after admission,cholinesterase activity of treatment group was significantly higher.The time of the control group's restoration of cholinesterase activity to 80% was significantly longer than that of treatment group(8.07 d±1.14 d vs 6.77 d±0.97 d,P0.05).The time of Atropinization was not significantly different;the incident of complications of treatment group was lower than that of control group(43.3% vs 16.7%,P0.05);length of hospital stay was also shorter than that of treatment group.Conclusion Ulinastatin is good for the restoration of cholinesterase activity.Ulinastatin can protect organ function and improve effectiveness of the treatment of acute severe organophosphorus pesticide poisoning.
