中西医结合治疗继发性肺结核的随机对照研究

摘　　要：目的:观察中西医结合治疗继发性肺结核的疗效。方法:对108例初治继发性肺结核随机分为实验组和对照组,两组均用2HREZ/4HR方案抗结核治疗,实验组根据辨证证型加用中成药服用6个月,肺阴虚证用滋阴润肺颗粒,阴虚火旺证用肺泰胶囊,气阴两虚证用双百口服液;每月复查血常规、肝肾功、痰涂片、胸片,隔月做中医证候评分。结果:完成疗程的90例病例(实验组48例,对照组42例),实验组病灶明显吸收率、总吸收率、中医证候治愈率和显效率明显高于对照组,而胃肠道反应明显少于对照组,均具有统计学意义(P<0.05)。实验组与对照组比较,患者潮热、盗汗、口渴等症状缓解较快。结论:中医药配合西药化疗,能迅速改善肺结核潮热、盗汗、口渴等症状;促进病灶吸收;减少抗结核药的不良反应发生率。Objective：To observe the curative effect of combining traditional Chinese and western medicine treatment of pulmonary tuberculosis in anti - tuberculosis period. Methods ： 108 cases of early cured secondary tuberculosis were ran- domly divided into experimental and control group, both groups with 2HREZ/4HR scheme antitubercular treatment, and the experimental group according to TCM syndrome differentiation added corresponding Chinese medicine for 6 months, the fei - yinxu syndrome with Ziyin Runfei particles, the yiuxu - huowang syndrome with Feitai capsule, the qiyin - liangxu syndrome with Shuangbai oral liquid. The blood - test, hepatorenal function, sputum smear and chest radiograph were monthly reexamined. The syndromes score was recorded every other month. Results ：90 cases accomplished the full course （the experimental group 48 cases and control 42 cases）. The nidus obvious absorption rate, total absorption rate, TCM symptomatic scores cure rate and significant effectual rate were higher than those in the control group, and gastrointestinal reaction was less than that of the control group, which all had statistical significance（ P 〈 0.05 ）. The experimental group compared with the control, symptoms of hot flash, night sweats, thirst and so on were relieved faster. Conclusion：Traditional Chinese medicine with western medicine chemotherapy could quickly improve tubeimulosis＇symptoms such-as hot flash, night sweats and thirst,promote nidus absorption,reduce the adverse reaction incidence of TB drugs.
