补脾益气升阳方治疗糖尿病肾病临床观察

摘　　要：目的:通过对DN患者尿中足萼蛋白、血管内皮生长因子变化的观察,探讨补脾益气升阳方对DN患者的作用机理,为临床应用提供理论依据。方法:60例糖尿病肾病患者随机分为治疗组和对照组,治疗组采用中药补脾益气升阳方,对照组采用辨证论治经验方,疗程为1个月,治疗前后分别观察患者的症状、体征、FPG、UAER、GFR、尿PCX、VEGF的变化。结果:临床观察发现,1个疗程后,治疗组中医症候疗效与对照组比较(P>0.05);治疗组空腹血糖﹙FPG﹚治疗前后比较(P<0.05),与对照组治疗后比较无统计学意义(P>0.05);治疗组肾小球率过滤﹙GFR﹚治疗前后比较(P<0.05),与对照组治疗后比较无统计学意义(P>0.05);24h尿蛋白﹙UAER﹚治疗前后相比有统计学意义(P<0.05);与对照组治疗后比较无统计学意义(P>0.05);治疗组尿PCX、VEGF治疗前后均有降低(P<0.05),与对照组相比较无统计学意义(P>0.05)。结论:补脾益气升阳方能够改善DN患者临床症状和体征,一定程度降低DN患者尿蛋白的含量及尿中PCX和VEGF的含量,提高肾小球滤过率。Objective ：To observe the changes of podocalyxin and vascular endothelial growth facto（VEGF） of DN pa- tients to discuss the mechanism of Bupi Yiqi Shengyang Formula treating DN and provide clinical basis. Methods ：60 dia- betes nephrosis patients were divided into treatment group and control group stochastically. The treatment group used Bupi Yiqi Shengyang Formula, the control group used the the proven prescription, observeing the symptoms and the related bio- chemistry target changes. Resuhs：A treatment course later, effectiveness in the treatment group and control group was re- spectively 86.67% and 83.33% , comparison between two groups had no statistics significance （P 〉 0.05 ）. The empty stomach blood sugar（FPG） in two groups after treatment had no statistics significance（ P 〉 0.05 ） ; renal glomerulus rate filtration（GFR） and 24h around the urine protein（UAER） in treatment group had statistics significance after treatment（P 〈 0. 05）. Conclusion ： Bupi Yiqi Shengyang Formula can improve clinical symptoms of diabetes nephrosis patients, reduce the albuminuria, enhance the renal glomerulus rate filtration.
