通腑泄浊法治疗腑实证慢性心力衰竭52例临床观察

摘　　要：目的观察通腑泄浊法治疗腑实证慢性心力衰竭(CHF)的临床疗效。方法将103例腑实证CHF患者随机分为2组,对照组51例予常规西药治疗,治疗组52例在对照组治疗基础上加通腑泄浊汤。2组均治疗7 d。观察2组治疗后喘憋、水肿、腹胀改善情况及心功能变化情况,记录2组治疗期间呋塞米日使用量,检测2组治疗前后血钾、血尿酸水平。结果治疗组喘憋、水肿及腹胀改善情况均优于对照组(P<0.05)。2组治疗后心功能均较本组治疗前明显改善(P<0.05),且治疗组改善优于对照组(P<0.05)。治疗组心功能改善总有效率96.1%,对照组心功能改善总有效率82.3%,2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。治疗组各患者呋噻米日用量明显少于对照组(P<0.05)。2组治疗后血钾与本组治疗前比较均明显下降(P<0.05),治疗组治疗后血钾水平高于对照组(P<0.05);治疗组治疗后血尿酸水平下降(P<0.05),对照组上升(P<0.05),且治疗组治疗后血尿酸水平低于对照组(P<0.05)。结论通腑泄浊法治疗腑实证CHF疗效满意,同时可以明显改善患者生活质量,减少利尿剂用量,改善电解质紊乱和高尿酸状态。Objective To observe the clinical effect of Tongfu Xiezhuo decoction on chronic heart failure with Fushi syndrome. Methods 103 patients with chronic heart failure with Fushi syndrome were randomly divided into two groups. 51 cases in control group were treated by conventional western medicine. 52 patients in treatment group were treated by Tongfu Xiezhuo decoction on the basis of conventional western medicine. The course was seven days in two groups. Improvement of wheezing,edema and abdominal distension and cardiac function changes were ob- served and the use of furosemide during the treatment, serum potassium levels before and after treatment was recor- ded. Results Improvement of wheezing,edema and abdominal distension in treatment group was superior to that in control group（P 〈0. 05 ）. Cardiac function after treatment was significantly improved as compared with that before treatment in two groups（ P 〈 O. 05 ）, the improvement in treatment group was superior to that in control group （P 〈 0.05）. The total effective rate of cardiac function in treatment group （96.1% ） was higher than that in control group （ 82.3 %, P 〈 0.05）. The serum uric acid levels after treatment decreased in treatment group （ P 〈 0.05）, the control group increased（ P 〈0.05 ）. The serum uric acid levels in treatment group was lower than that in control group after treatment（P 〈 0.05 ）. Conclusion Tongfu Xiezhuo decoction were satisfactory on chronic heart failure with Fushi syndrome, and can significantly improve patients＇quality of life, reduce the diuretic dosage to reduce the state of elec- trolyte imbalance, and high uric acid.
