摘  要:目的:观察应激性胃溃疡大鼠模型中Th17细胞功能的变化,并观察乌司他丁对这一过程的干预作用。方法:SD大鼠32只,随机分为4组:正常组(N组)、应激组(S组)、小剂量乌司他丁组(U1组)和大剂量乌司他丁组(U2组)。S、U1组和U2组经过预处理后进行束缚浸水应激8h。采用荧光定量PCR检测脾脏Th17细胞相关转录因子RORγt和分泌因子IL-17AmRNA水平,采用ELISA试剂盒检测血浆IL-17A蛋白水平,并检测血浆丙二醛(MDA)和超氧化物歧化酶(SOD)水平。另外,取胃观察胃溃疡指数(ulcer index,UI)及组织病理变化。结果:与N组比,S组SOD降低,MDA、UI、RORγt mRNA和IL-17A水平均升高(P<0.05);与S组比,乌司他丁用药组(U1和U2组)SOD升高,MDA、UI、IL-17A都有不同程度的降低,而RORγt mRNA无明显变化。结论:Th17细胞参与了束缚浸水应激引起的胃黏膜损伤,乌司他丁能够减轻胃黏膜损伤,其作用机制可能与抑制了IL-17引起的炎症级联反应有关。Objective To explore the effect of ulinastatin on Thl7 cell function in the rat model of stress- induced ulcer. Methods Thirty-two male Sprague Dawley rats were randomly divided into control group (group N), stress ulcer model group (group S), group of ulinastatin 50 kU/kg (group U1), and group of ulinastatin 100 kU/kg (group U2). After pretreatment, the latter three groups were subjected to water-immersion and restraint stress for 8 hours, mRNA expressions of RORγt and IL-17A in spleen Th17 cells were measured by RT-PCR. Level of plasma IL-17A was detected by ELISA, and superoxide dismutase (SOD) activity and malondialdehyde (MDA) level were assayed. Ulcer index (UI) was determined and pathological changes in the stomach were observed. Results As compared with group N, SOD activity decreased while levels of MDA, UI, RORγt mRNA, and IL- 17A increased in group S (P 〈 0.05). As compared with group S, SOD activity elevated while levels of MDA, UI, and IL-17A declined in groups U1 and U2, but the expressions of RORγt mRNA remained unchanged. Conclusions Thl7 cells are involved in the development of gastric mucosal damage induced by water-immersion and restraint stress. Ulinastatin can alleviate gastric mucosal injury', whose mechanism may be related to the inhibition of inflammatory cascade caused by IL-17.
