术前应用颈托训练对颈椎前路手术术后佩戴颈托依从性的影响

摘　　要：目的观察术前应用颈托训练对颈椎前路手术术后佩戴颈托的依从性的改善情况。方法对84例择期拟行颈椎前路手术的患者,按入院顺序随机分组,实验组(n=42)入院日至手术日每日佩戴颈托6h,并由主管护士指导患者佩戴颈托下进行翻身、进食、如厕训练;对照组(n=42)术后开始佩戴颈托。术后对患者佩戴舒适性主诉分级记录(3分,无不适主诉;2分,不适可耐受;1分,难以耐受但可接受单片固定;0分,不能佩戴),并依据护理巡视记录颈托佩戴情况(每次检查佩戴可靠计1分,拆卸计0分),每小时记录1次,至术后第3天结束。结果两组患者舒适性评分差异无统计学意义;术后佩戴颈托的依从性评分,实验组显著优于对照组(P<0.01)。结论术前应用颈托训练对颈椎前路手术患者术后舒适性提高无明显作用,但对术后颈托佩戴的依从性具有良好的提高作用,良好的术后颈部制动对术后恢复具有积极作用。Objective To improve the compliance of postoperative gear wearing in patients with anterior approach cervical surgery by preoperative training. Methods From Nov 2010 to May 2011, 84 patients underwent anterior ap- proach cervical surgery were divided into two group randomly according to hospital admission order： experiment group （n= 42） and control group （n=42）. In experiment group, patients must wear cervical gear 6 h every day before surgery, and were trained by one nurse in charge to do daily activities with cervical gear, such as turning the body over, taking food, or going to toilet, etc. While in control group, all patients did not wear cervical gear before cervical surgery. The degree of all pa- tients＇ compliance were scored （3,no complain scored; 2,not comfortable but tolerable; 1, tolerable for just single brace; 0, not tolerable） and the cervical gear wear condition were recorded every hour for 3 days. The data were analyzed with non-parameter analysis. Results There was no difference between two groups in pleasance （P 〉 0.05）, while there was statistical difference between two groups in compliance （P 〈 0.01）. Conclusion Preoperative training does not improve the degree of comfort, while it can improve the compliance significantly, which brings a positive effect for postoperative reha- bilitation.
