舒芬太尼与芬太尼静脉镇痛对胃癌根治术后患者T细胞亚群的影响

摘　　要：目的比较舒芬太尼与芬太尼静脉镇痛对胃癌根治术后患者T细胞亚群功能的影响。方法择期行胃癌根治术的胃癌患者60例,随机分为,全麻术后分别行舒芬太尼镇痛组和芬太尼静脉镇痛组,分别于麻醉前、术后1d、2 d和3 d从右颈内静脉抽取中心静脉血1 ml,检测T细胞亚群水平;分别于术后4 h、术后24 h、术后48 h和术后72 h比较两组患者视觉模拟评分(VAS),比较两组术后72 h内不良反应情况。结果与芬太尼组比较,舒芬太尼组术后2 d时CD3+、CD4+和CD4+/CD8+水平明显较高(P<0.05);术后72 h内两组患者VAS评分比较差异无统计学意义(P>0.05);两组患者不良反应发生率比较差异明显(P<0.05)。结论舒芬太尼对机体免疫功能具有一定保护作用,更适用于胃癌根治术患者术后康复。Objective To compare the effects of Sufentani and Fentanyl intravenous analgesia on the T-lymphocyte subsets in patients with gastric cancer after radical operation.Methods 60 cases undergoing radical gastrectomy for gastric cancer were randomly divided into two groups who would be given the intravenous analgesia of Sufentanil and Fentanyl after general anesthesia.The 1ml of blood were extracted from deep vein under the right subclavian test the levels of T-lymphocyte subsets before anesthesia,postoperative1d,2d and 3d.We should compare the visual analogue scale（VAS） scores of two groups respectively within 4h,24h,48h and 72h after operation and analyze the adverse reaction between Sufentanil group and Fentanyl group within 72h after operation.Results Compared with the Fentanyl group,the amount of CD3＋,CD4＋ and CD4＋/CD8＋ in Sufentanil group were significantly higher（P0.05） within 2d after operation.The VAS scores of the two groups had no difference within 72h after operation（P0.05）.The adverse reaction between Sufentanil group and Fentanyl group were significantly different（P0.05）.Conclusions Sufentanil is playing an important role in protecting immune functions and more suitable for radical surgery for gastric cancer patients after operation.
