摘  要:目的采用大鼠暂时性肝门阻断模型,分别监测门静脉分流和阻断后大鼠缺血再灌注后肝脏功能的变化,为临床上选取手术方式以减轻肝缺血再灌注损伤提供参考。方法将大鼠分为假手术组、实验组和对照组3组,制作门静脉转流下大鼠肝脏缺血再灌注模型,观察大鼠的一般情况及术后1、6、24h及3、7d肝脏的形态功能变化。结果①实验组术中大鼠胃肠及脾脏无淤血,术后大鼠恢复时间较短,而对照组大鼠可见脾脏明显淤血,术后大鼠精神差,恢复时间延长。②术后1、6、24h及3、7d,实验组大鼠血清ALT及AST值均明显低于对照组(P<0.05)。③实验组在各时相点肝组织损伤均较对照组轻。结论肝门阻断时门体分流处理能够改善肝功能,提高缺血再灌注后肝脏的抗损伤能力。Objective A model of ischemia reperfusion injury of rat liver with portal-cervical bypass was established in this experiment to compare different effect on rats liver function between the portal-cervical bypass and interruption of portal vein, which provides a reference for clinical operation manner to reduce ischemia reperfusion injury in liver surgery, Methods A model of ischemia reperfusion injury of rat liver with portal-cervical bypass was established in this experiment to compare different effect on rats liver function among the sham operation group, experimental group and control group. Results Rats without obvious congestion of vein system and the liver color recover soon with renew blood supply in experimental group. Whereas, that with marked congestion of gastrointestines and spleen had poor spirit, lon- ger recovery in control group. Contrast to the experimental group, the hepatic cells in the control group with partially de- stroyed hepatic lobule structure appeared extensive denaturalization and necrosis after the same time of operation, and these more severe pathological changes alleviate and recover after 24 hours, 3 days, and 7days respectively. Conclusion The current results show that employing portocaval shunt method may improve liver function of rat liver after operation.
