摘 要:目的探讨MRI对肝腺瘤的诊断价值。方法回顾性分析10例肝腺瘤患者的MRI表现。结果病理证实肝腺瘤10例,其中MRI诊断为肝腺瘤5例。7例单发,直径为(5.2±1.9)cm;3例多发,最大病灶直径为(9.0±3.0)cm。8例T1WI序列和T2WI序列呈长T1和长(稍长)T2信号,1例呈长T1稍短T2信号,1例呈短T1等T2信号;9例回波序列见大量脂质成分。增强扫描时,7例动脉期病灶明显强化,门脉期及延迟期造影剂消退,病灶为等信号或稍高信号;2例动脉期轻度不均匀强化;1例动脉期病灶增强与肝实质近似,门脉期和延迟期造影剂较前消退。肝腺瘤呈现"快进慢出"信号特征;8例门脉期及延迟期病灶周围可见环形强化假包膜。结论肝腺瘤的增强MRI有特征性表现,有助于与其他肝内肿瘤鉴别,但明确诊断仍须要病理支持。Objective To investigate the value of magnetic resonance imaging (MRI) in the diagnosis of hepatic adenoma. Methods MRI findings of 10 cases of hepatic adenoma confirmed by histopathology and undergoing multiphase contrast-enhanced MRI, were analyzed retrospectively. Results Among 10 eases of hepatic adenoma confirmed by histopathologieal examination, only 5 eases were diagnosed with hepatic adenoma by MRI. Seven cases had solitary lesion with the average diameter of 5.2±1.9 cm, and the other 3 had multiple lesions with the average diameter of 9.0±3.0 cm. Eight cases showed long T1 and (slightly) long T2 signals on T1W1 and T1W2 sequence imaging, 1 case showed long T1 and shorter T2 signals, and 1 case showed shorter T1 and the same T2 signals. Dual-echo sequenees of 9 eases showed a large number of lipid. For enhanced-MRI scanning, 7 eases showed signifi- cantly enhanced lesions in arterial phase, and gradual reduction in the effect of contrast agents in portal venous phase and delayed phase while lesions showed isointense or slightly hyperintense; 2 eases showed mildly heterogeneous enhancement in arterial phase; 1 case showed isointense with sourrounding liver in arterial phase and gradual reduction in the effect of contrast agents in portal venousphase and delayed phases. Hepatic adenoma had "fast-in and slow-out" pattern. Eight eases showed intensified pseudoeapsule ring in portal venous phase and delayed phase. Conclusions Enhanced-MRI findings of hepatic adenomas help to distinguish hepatic adenomas from other intrahepatic tumors, but a definitive diagnosis of hepatic adenomas can only be made through histopathological analysis.
