无锡市社区高血压病患者控制状况及相关因素分析

摘　　要：目的评估社区高血压病患者的控制状况,分析影响血压控制效果的相关因素,为寻求针对性的高血压病强化干预措施提供依据。方法通过分层整群抽样方法,对抽中社区在社区卫生服务中心登记管理的30～70岁的2 675例高血压病患者进行流行病学问卷调查、体格检查和血生化检查。结果无锡市社区管理的30～70岁高血压病患者的达标率为35.6%,其中城市43.1%,农村27.0%,多因素Logistic回归分析显示,文化程度高、每周体育锻炼时间≥120 min、高血压病防治知识得分高等为血压控制达标的有利因素;而每日食油量多、高血压病程长、空腹血糖和总胆固醇水平高为血压控制达标的不利因素。结论以社区为基础的高血压病规范化管理显著提高了血压控制达标率,社区医护人员在管理高血压病患者过程中应重视对饮食、运动指导、降脂以及高血压病防治知识的宣传。Objective To evaluate the status of blood pressure control among hypertensive patients managed by community health service center in Wuxi City and to explore the relevant factors. Methods Stratified cluster random sampling was used and 2 675 hypertension patients aged from 30 to 70 in 4 communities which were managed by community health service centers were surveyed with questionnaires, physical and biochemical examinations. Results The overall attainment rate of blood pressure control was 35.6% among hypertension patients aged from 30 to 70 managed by community health service center in Wuxi City. The attainment rates were 43.1% and 27.0% in urban and rural area respectively. The results of muhi-variate Logistic regression showed that high level of education, physical exercise for more than 120 minutes per week, high score in knowledge of preventing and treating hypertension were helpful to blood pressure control, whereas high oil intake, long course of disease, high level of fasting blood glucose and total cholesterol were unfavorable factors. Conclusions Regular management based on community health service greatly improves the attainment rate of blood pressure control. Medical workers should pay more attention to dietary and exercise guidance, lipid-lowering and publicizing prevention and control knowledge of hypertension.
