摘  要:【目的】探讨激活补体是否会引起心肌缺血损伤 ,以及膜结合型补体调节蛋白CD59对补体引起的心肌缺血损伤有无保护作用。【方法】 18只豚鼠离体工作心脏随机分为 :A组 (对照组 ) ,在灌注液中加入体积分数为 3%灭活人血浆 +酵母多糖 ;B组 ,在灌注液中加入 3%人血浆 +酵母多糖 ;C组 :在灌注液中加入CD59(45 0 μg) +3%人血浆 +酵母多糖。记录各组处理前及处理后 15、30、45和 6 0min的心外膜心电图ST段变化、冠脉流量 (CF)、心输出量 (CO)、左心室最大内压 (LVPmax)、左心室内压最大上升速率 (dp/dtmax)和心率 (HR)。【结果】A组处理前后各项观察指标无明显变化 ;B组在接受处理后心外膜心电图出现ST段抬高、HR增加 ,CF、CO、LVPmax和dp/dtmax均下降 ,这些变化以处理后 45min最明显 ;C组除处理后 45~ 6 0min时心率稍增快外 ,其余指标处理前后无明显变化。【结论】 激活补体可直接引起豚鼠工作心脏心肌缺血损伤 ,CD59对补体介导的心肌缺血损伤有保护作用。Objective To observe that activated human complements directly injure isolated quinea pig working hearts and CD 59 prevents it. Methods Isolated guinea pig working hearts perfused with 3% denatured human plasma and zymosan in group A (control), 3% normal human plasma and zymosan in group B and 3% normal human plasma and CD 59 (450 μg) and zymosan in group C, respectively. Of these 3 groups, epicardial electrocardiogram (ECG) and coronary flow(CF), cardiac output(CO), maximum left ventricular developed pressure (LVP max ),maximum left ventricular pressure rise rate (dp/dt max ) and heart rate(HR) were recorded before and after 15, 30 , 45 and 60min perfusion. Results No apparent change in all above parameters was seen ingroup A before and after perfusion ;elevation of ST segment,faster HR and decrease in CF, CO, LVP max and dp/dt max were observed in group B, most prominent at 45 min after perfusion; only slight increase in HR in all observed parameters was observed in group C at 45~60 min after perfusion. Conclusion Activated human complements directly injured isolated guinea pig working hearts and CD 59 prevents it.
