摘 要:目的:探讨320排CT头颈部血管成像的扫描方法及其对头颈部血管病变的临床诊断价值。方法:152例疑似头颈部血管病变患者行320排CT血管成像,扫描数据调入Vitrea fx后处理工作站进行后处理,其中21例同时行数字减影血管造影术(DSA)。结果:该组152例320排CTA检查均能清楚显示头颈部动脉,其中,阴性35例,动脉闭塞15例,动脉变细或狭窄28例,动静脉畸形8例,动脉瘤23例,动脉钙化或斑块76例。21例同时行DSA检查,其中,320 CTA检出的5例动脉狭窄仅1例得到DSA证实,余4例中3例为阴性,1例为颅底小动脉瘤;320 CTA检出的9例动脉瘤经DSA检查证实均有动脉瘤存在;320 CTA检查中3例阴性病例DSA检查也为阴性。2种检查结果基本相符。结论:320排CT头颈部血管成像可清楚显示头颈部血管病变,避免了分段成像的遗漏或不必要的重复及追加扫描,为头颈部血管疾病的诊断提供了无创性"一站式"检查方法。Objective To investigate 320-row CT angiography and its clinical value for the vascular diseases of head and neck.Methods There were 152 patients suspected with vascular diseases examined by 320-row CT angiography,and the scanning data were input into Vitrea fx post-operation workstation,of which,21 ones also underwent DSA examination.Results All the 152 cases had their vessels of head and neck displayed clearly,of which,35 ones were found with negative results,15 ones with arterial occlusion,28 ones with artery tapering or narrowing,8 ones with arteriovenous malformation,23 ones with arteriovenous aneurysm,and 76 ones with arteriosteogenesis or arteriosteogenesis.In the 21 cases undergoing DSA examination,there were 5 ones determined by 320 CTA with arteriovenous aneurysm,of which,1 case was proved,3 ones were found with negative results,and 1 case with basilar microaneurysm.Nine ones were diagnosed with arteriovenous aneurysm and 3 ones without positive results by 320 CTA and DSA.The results by the two examination methods were compatible.Conclusion Vascular diseases of head and neck can be diagnosed by 320-row CT angiography,with the additional scanning avoided.
