腔内短波理疗联合针刺对体外受精-胚胎移植子宫内膜容受性的影响

摘　　要：目的观察腔内短波理疗联合针刺对体外受精-胚胎移植(IVF-ET)患者子宫内膜容受性的影响。方法将97例IVF-ET患者随机分为腔内短波理疗联合针刺组(51例)和单纯西药组(46例),两组患者均采用黄体期长方案,在超排卵的同时,腔内理疗联合针刺组在月经干净后,每天进行理疗和针刺,直至注射绒毛膜促性腺激素(HCG)日。测定两组患者HCG日子宫内膜厚度、分型及子宫内膜血流阻力指数(RI),并观察两组患者取卵数、优卵率、受精率、优胚率以及临床妊娠率。结果子宫内膜厚度两组比较差异无统计学意义(P>0.05);A型内膜腔内短波理疗联合针刺组所占比例为74.5%,单纯西药组为45.7%,两组比较差异有统计学意义(P<0.01);卵泡膜血流RI腔内短波理疗联合针刺组明显低于单纯西药组(P<0.01);两组患者取卵数、受精率差异无统计学意义(P>0.05);优卵率、优质胚胎率及临床妊娠率腔内短波理疗联合针刺组均高于单纯西药组(P<0.05)。结论腔内短波理疗联合针刺能明显提高IVF-ET患者子宫内膜容受性,提高临床妊娠率。Objective To observe the effect of intracavitary shortwave physiotherapy combined with acupuncture on the endometrial receptivity of in vitro fertilisation and embryo transplantation （IVF-ET） patients and evaluate the treatment outcome. Methods Ninety seven IVF-ET patients were randomized into combined treatment group of 51 cases and western medicine group of 46 cases. Both groups were given [uteal phase long protocol. During the controlled ovarian hyperstimulation, the com- bined treatment group was given acupuncture combined with intracavitary shortwave physiotherapy after menstruation and las- ted to the day of injecting human chorionic gonadotropin （HCG）. The western medicine group was given western medicine treatment only. The endometrial thickness, type an3 endometrial blood flow resistance index （RI） on the day of injecting HCG and the number of retrieved oocytes, rate of high-quality oocytes, fertilization rate, rate of high-quality embryos and clinical pregnancy rate were observed and compared. Results There was no significant difference in the endometrial thickness between groups （P〉0.05）. There was 74.5% （38/51 cases） of A-type endometrium in the combined treatment group and 45.7% （21/ 46 cases） in the western medicine group, with significant difference between groups （P〈0.01）. The blood flow RI of follicle membrane on the day of injecting HCG in the combined treatment group was significantly lower than that in the western medi- cine group （P〈0.01）. There was no significant difference in the number of retrieved oocytes and the fertilization rate between groups （P〉0.05）. The rate of high-quality oocytes, high-quality embryos and clinical pregnancy rate in the combined treat- ment group were significantly higher than those in the western medicine group （P〈0.05）. Conclusion Intracavitary shortwave physiotherapy combined with acupuncture could obviously improve the endometrial receptivity of IVF-ET patients and im- prove pregnancy rate.
