NICU产超广谱β-内酰胺酶肠杆菌定植的前瞻性研究

摘　　要：目的了解新生儿重症监护病房(NICU)住院患儿产超广谱β-内酰胺酶(ESBLs)肠杆菌定植状况,有效预防与控制医院感染的发生。方法连续采集住院患儿不同时期直肠拭子进行产ESBLs显色平板筛选试验,同时收集每例患儿临床及试验室辅助检查资料。结果共监测患儿31例,入院第1、4、11天产ESBLs肠杆菌定植率分别为29.0%、70.0%、84.2%,均为产ESBLs肺炎克雷伯菌和产ESBLs大肠埃希菌,住院第4天患儿产ESBLs肠杆菌定植呈明显上升趋势(P<0.05)。结论产ESBLs肠杆菌在NICU患儿直肠定植率随着住院时间的延长而显著增加,应缩短住院天数,合理使用抗菌药物,做好消毒隔离,防止产ESBLs肠杆菌在院内暴发流行。OBJECTIVE To understand the status of colonization of extended spectrum β-lactamases（ESBLs）-producing Enterobacteriaceae isolated from the newborn intensive care unit（NICU）,so as to effectively prevent and control the incidence of nosocomial infections.METHODS The rectal swab samples were continuously collected from the hospitalized children in various periods and were inoculated directly to ChromID ESBLs color plate for ESBLs-produing Enterobacteriaceae screening test,meanwhile,the clinical data and the laboratory secondary data of each child were collected.RESULTS Of totally 31 children investigated,the colonization rates of ESBLs-producing Enterobacteriaceae on the 1 st,4 th,and 11 th day of the admission to the hospital were 29.0%,70.0% and 84.2%,respectively,the majority of ESBLs-producing Enterobacteriaceae were ESBLs-producing Klebsiella pneumoniae and ESBLs-producing Escherichia coli,the colonization of ESBLs-producing Enterobacteriaceae kept a significant upward tendency 4 days after the admission（P0.05）.CONCLUSION The colonization rate of ESBLs-producing Enterobacteriaceae is increased significantly as the duration of hospitalization extends,which indicated that the hospital stay need to be shortened,the antibiotics should be used reasonably,and the disinfection and isolation should be performed well so as to prevent the outbreak and spread of ESBLs-producing Enterobacteriaceae in the hospital.
