厚朴粉敷贴对腹部手术后排气促进作用的临床研究  

Influence of Point Application of Cortex Magnoliae Officinalis Powder on Promoting Aerofluxus in Patients After Abdominal Operation

摘　　要：【目的】了解厚朴粉穴位敷贴足三里促进腹部术后患者胃肠功能恢复、促进排气排便的疗效及安全性。【方法】将符合纳入条件的448例患者随机分为穴位敷贴组和空白对照组,穴位敷贴组在术后30 min内即给予厚朴粉穴位敷贴双侧足三里,每6 h更换1次药物,直至患者排气为止;空白对照组不给予穴位敷贴。观察2组患者排气排便的时间和肠鸣音等。【结果】术后排气时间、术后肠鸣音的全分析集(FAS)和符合方案集(PPS)的分析结果一致,均显示差异有统计学意义(P<0.05或P<0.01),表明通过穴位敷贴厚朴粉,能明显改善胃肠功能,促进肠蠕动,促进肠鸣音恢复正常和排气。而厚朴粉穴位敷贴足三里不能改善排便时间,2组比较差异无统计学意义(P>0.05)。【结论】穴位敷贴足三里能明显恢复胃肠功能,促进肠鸣音恢复正常,促进排气,且具有良好的安全性,值得临床推广运用。Objective To investigate the therapeutic effect and safety of point application of Cortex Magnoliae Officinalis powder on promoting gastrointestinal function recovery, aerofluxus and defecation in patients after abdominal operation, and to evaluate the safety. Methods The qualified 448 patients were randomized into point application group and blank control group. The point application group was given application of Cortex Magnoliae Officinalis powder on bilateral Zusanli （ST36） 30 min after abdominal operation, and the powder was changed once every 6 hours till aerofluxus occurred. The blank control group had no point application. The time for first aerofluxus and defecation after operation, and the bowel sound were observed in both groups. Results The results of full analysis set （FAS） and per protocol set （PPS） showed that the differences of bowl sound and postoperative aerofluxus time were significant between the two groups （ P 〈 0. 05 or P 〈 0. 01 ）, indicating that point application of Cortex Magnoliae Officinalis powder could obviously improve gastrointestinal function, intestinal peristalsis, bowel sound and aerofluxus. However, point application of Cortex Magnoliae Officinalis powder had no effect on time for first defecation, the difference between the two groups being insignificant （P 〉 0. 05 ） Conclusion Point application of Cortex Magnoliae Officinalis powder on Zusanli is effective and safe on promoting the recovery of gastrointestinal function, bowel sound and aerofluxus.
