摘  要:目的探讨高血压患者血压晨峰与心率变异性(HRV)的相关性。方法对56例高血压患者行24小时动态血压监测的同时行24小时动态心电图监测。根据血压晨峰情况分为两组:晨峰组(31例),血压晨峰为起床后2小时内平均收缩压与睡眠时最低平均收缩压(最低收缩压及其前后2次血压的平均值)之间的差值≥55mmHg,其他为非晨峰组(25例)。分析24小时动态心电图计算机自动给出心率变异性参数的关系。结果晨峰组SDNN、SDANN、SDNN、PNN50与非晨峰组间差异有统计学意义。结论具有高血压晨峰现象的患者心率变异性偏低,提示交感、迷走神经功能平衡降低,心室肌电不稳定性增加。Objective To evaluate the relationship between morning blood pressure surge and heart rate variability(HRV)in the patients with hypertension.Methods Fifty-six patients with hypertension were performed by 24-hour ambulatory blood pressure monitoring.They were divided into with morning blood pressure surge group(31cases) and without morning blood pressure surge group(25 cases) according to the condition of 24-hour ambulatory blood pressure monitoring.The deviation of the average value in awaking 2 hours and the minimum value during sleep of the systolic blood pressure no less than 55 mmHg was defined as morning blood pressure surge.Their HRV time domain indexes were analyzed.Results The SDNN,SDANN,SDNN,PNN50 of morning blood pressure surge group were significantly higher than without morning blood pressure surge group.Conclusion The autonomic nerve function was damaged and electrophysiological instability of ventricular myocardium increased significantly in hypertension patients with morning blood pressure surge.
