风湿性二尖瓣狭窄患者左房内自回声现象的血液流变学机制

摘　　要：探讨风湿性二尖瓣狭窄患者左心房血液流变学特点及与左心房自回声现象（ ＳＥＣ）的关系。９９例风湿性二尖瓣狭窄患者于经皮球囊二尖瓣成形术（ Ｐｒｅｃｕｔａｎｅｏｕｓ Ｂａｌｌｏｎ Ｍｉｔｒａｌ Ｖａｌｖｕｌｏｐｌａｓｔｙ， ＰＢＭＶ）前采用 ＨＰ ＳＯＮＯＳ２５００型彩色多普勒成像仪（探头频率为５ＭＨｚ）行食道超声检查，术中自左心房取血液标本进行血液流变学检查。结果表明：心房颤动、二尖瓣狭窄程度、左心房直径、左心房压力均 ＳＥＣ的独立预测因子。ＳＥＣ组Ｈｃｔ、ηｐ、 ηａ（ Ｌ）、 ηａ（ Ｈ）、 ＩＲＣＡ、 Ｔｋ均高于无 ＳＥＣ组（ｐ＜０．０５或＜ ０．０１）。提示风湿性二尖瓣狭窄患者ＳＥＣ的形成与血液流变学异常存在密切关系。The purpose of this study was to observe the relationship between left atria hemorheological property and left atria spontaneous echo contrast(SEC) in rheumatic mitral stenosis. The clinical and echocardiographic variables related to SEC were evaluated in a series of 99 patients undergoing percutaneous balloon mitral valvuloplasty and transesophageal echocardiography with a 5 -- MHz single plane transducer. SEC was found in 32 patients with rheumatic mitral stenosis (32. 32% ). Multiple logistic regression analysis showed that the severity of atrial fibrillation, mitral stenosis, diameter of left atrium, pressure in left atrium and age were the independent predictors of SEC. Hct. p. a(L) . a(H) , IRCA and Tk were higher in rheumatic mitral stenosis with SEC than without SEC. It suggested that SEC is associated with hemorheologic abnormalities in left atrium.
