支撑喉镜术使用常频喷射通气的观察

摘  要:目的 :比较常频喷射通气和高频喷射通气在支撑喉镜术中的通气效果。方法 :选择 32例美国麻醉协会 (ASA)五级分类法Ⅰ~Ⅱ级声带息肉患者 ,拟行支撑喉镜术 ,随机分为高频组和常频组 ,每组 16例。结果 :采用 6 0次 /分的频率 ,16 0kPa压力进行通气 ,术中PaCO2 明显升高 (P <0 .0 1) ,并形成高碳酸血症 ;采用 2 2次 /分的频率、16 0kPa的压力进行通气 ,术中PaCO2 仅有升高趋势 ,但差异无显著性 (P >0 .0 5 ) ,且其水平明显低于高频组 (P <0 .0 1)。两组患者术中的PaO2 均明显升高 ,但两组间差异无显著性 (P >0 .0 5 )。结论 :无论是常频还是高频喷射通气 ,都能满足机体对供氧的需要 ;而常频通气比高频通气更有利于CO2 排出。认为支撑喉镜术宜采用常频喷射通气。?Objective:To compare the effect of the normal frequency jet ventilation with the high frequency jet ventilation in suspension layngoscopy.Methods:Thirty two(ASA grade Ⅰ~Ⅱ ) patients with vocal polyp undergoing laryngoscopy were randomly divided into two groups:high and normal frequency groups.Either group had 16 patients.Results:The PaCO\-2 increased significantly( P <0.01) and hypercapnia occurred during the ventilation with frequency of 60 bpm and driving pressure of 160 kPa;but,PaCO\-2 increased slightly( P >0.05) during the ventilation with frequency of 22 bpm and driving pressure of 160 kPa the extent of the increasing was much less than that of high frequency group( P <0.01).The PaO\-2 increased after jet ventilation and the difference increase between the 2 groups was not signifcant( P >0.05).Conclusions:The normal frequency jet ventilation serves as breath support during laryngoscopy because of its advantage of CO\-2 excretion over that in high frequency jet ventilation.
