新生儿肠外营养相关性胆汁瘀积症的临床分析

摘  要:目的探讨新生儿长期胃肠外营养相关性胆汁瘀积症的危险因素,为临床的防治提供参考。方法回顾性分析2010年1月至2012年1月收治的长期胃肠外营养的新生儿患者168例,其中发生胃肠外营养相关性胆汁瘀积症( parenteral nutrition associated cholestasis, PNAC )患儿131例作为实验组,未发生胃肠外营养相关性胆汁瘀积症患儿137例作为对照组,实验组和对照组进行单因素非条件和多因素非条件的Logistic回归分析。结果单因素分析结果显示PN持续天数、氨基酸的累积用量、葡萄糖的累积用量、脂肪乳的累积用量、是否感染及是否早产,差异均有统计学意义(P〈0.05)。多因素非条件的Logistic回归分析显示PN持续天数、氨基酸和脂肪的累积用量、是否感染及是否早产是新生儿长期胃肠外营养相关性胆汁瘀积症的独立危险因素。结论在使用PN的过程,尽早给予胃肠内喂养,缩短PN时间,注意防治感染,减少脂肪乳、氨基酸的累积用量等,可能是减少PNAC发生的有效措施。Objective To explore risk factors of neonatal long-term parenteral nutrition correlation cholestasis, and it provided the reference for clinical. Methods Long-term parenteral nutrition in 168 cases of newborn babies were retrospective analyzed from January 2010 to January 2012 were, which occur parenteral nutrition associated cholestasis (PNAC) 31 cases as the experimental group, did not happen parenteral nutrition correlation cholestasis disease 137 cases of children as control group, the experimental group and control group have single factor and many factors of Logistic regression analysis. Results Single factor analysis results show that for days, amino acids of PN accumulated dosage, the accumulation of glucose, dosage of fat emulsion cumulative amount, infection and premature, the difference was statistically significant ( P〈0.05 ) . Many factors of Logistic regression showed that PN last days, amino acids and adipose accumulation dosage, infection and premature babies is long-term parenteral nutrition correlation cholestasis independent risk factors for the disease. Conclusion In the process of PN use, to give early intestine fed, to shorten the PN time, to pay attention to control infection and to reduce the accumulation of fat emulsions or amino acids may be the effective measures to reduce PNAC.
