微血管减压术治疗老年面肌痉挛的临床疗效和安全性分析

摘　　要：目的探讨微血管减压术（MVD）在老年患者面肌痉挛治疗上的临床疗效和安全性。方法收集自2005年至2012年351例因面肌痉挛在大连大学附属中山医院行MVD的患者，将其中44例年龄大于等于65岁为老年组，余307例年龄小于65岁为年轻组。回顾性分析比较老年组和年轻组的治愈率及并发症。结果2组患者一般资料比较：老年组与年轻组患者在男女比例、病变部位、住院时间、责任血管上差异无统计学意义（P〉0．05）；在年龄、病程上差异有统计学意义（P〈0．05）。老年患者总体治愈率为97．7％，年轻组为97．1％；并发症如永久性神经功能障碍，导致听力下降和面肌麻痹在老年组有1例（占2．27％，1／44），年轻组为5例（占1．63％，5／307），无死亡病例。结论MVD是治疗面肌痉挛最有效的治疗方法，治疗老年面肌痉挛患者较之年轻患者没有更高的并发症发生率和死亡率。对于原发性面肌痉挛患者，只要其一般健康情况能够耐受全身麻醉，可考虑行MVD。Objective To investigate the curative effect and safety of microvascular decompression （MVD） for hemifacial spasm （HFS） in elderly patients. Methods Three hundred and fifty-one patients with HFS, admitted to our hospital from 2006 to 2011 and underwent MVD, were chosen in our study; among them, 44 patients older than 65 years were chosen as elderly group and the other as young group. The complications and the cure rates were evaluated and compared between the elderly group and the young group. Results No significant statistical differences on gender, location of the lesions, hospitalization time and offending vessels were noted between the elderly and young groups （P〉0.05）. However, significant statistical difference on both age and diease duration was noted between the two groups （P〈0.05）. The overall cure rate was 97.7% for elderly patients and 97.1% for young patients. Permanent neurological deficits, such as hearing loss and facial paralysis appeared in one of the elderly patients （2.27%, 1/44） and 5 patients of the young group （1.63%, 5/307）. No death case was noted. Conclusion MVD is the most effective operation treatment for HFS; no higher rate of complications and mortality in treating elderly patients with HFS is conformed as compared with that in young patients; any patients with HFS, as long as their general health condition can tolerate general anesthesia, are supposed to undergo MVD.
