摘  要:目的考察胶原蛋白支架-硅胶膜双层敷料修复深度烧伤创面的效果。方法将24例深度烧伤患者随机分为治疗组和对照组,每组12例。治疗组采用胶原蛋白支架-硅胶膜双层敷料覆盖创面10~20d后,清除硅胶膜,移植自体刃厚皮。对照组直接移植自体刃厚皮覆盖创面。考察两组患者术后3d创面疼痛程度、创面愈合时间,评估瘢痕形成和增生程度,并观察创面愈后瘢痕挛缩率。结果治疗组患者术后3d创面疼痛评分(3.25±1.06),低于对照组(4.83±1.12),差异有统计学意义(P<0.05);治疗组平均愈合时间为(40.08±4.54)d,较对照组(52.17±6.00)d缩短,差异有统计学意义(P<0.05)。愈后3个月和6个月,治疗组创面瘢痕程度VSS评分(7.00±1.21)和(4.25±0.97),低于相同时间点对照组VSS评分(10.92±2.28)和(8.42±2.15),差异有统计学意义(P<0.05)。治疗组的瘢痕发生率33.33%较对照组75.00%低,差异有统计学意义(P<0.05)。结论胶原蛋白支架-硅胶膜双层敷料能够促进深度烧伤创面修复和真皮再生,改善创面瘢痕愈合状况,临床疗效良好。Objective To observe the effect of collagen scaffold and silica gel membrane double-layer dressing (CS-SGM-D) on repairing deep burn wound. Methods Twenty-four patients of deep burn wound were divided into treatment group (T-group) and control group (C-group), each with 12 cases. CS-SGM-D was applied to cover the wound firstly in T-group. Ten to 20 days later, the silica gel membrane was cleared, and split-thickness skin autograft was transplanted, whereas split-thickness skin autograft was transplanted directly in C-group. Three days after surgery, wound pain level and wound healing time were observed. Scar formation and proliferation were evaluated. Morbidity rate of scar contracture was observed. Results Three days after surgery, wound pain grade was 3.25±1.06 in T-group, lower than C-group, 4.83±1.12, and the difference was statistically significant (P0.05). Average wound healing time in T-group was 40.08±4.54 days, shorter than in C-group, 52.17±6.00 days, and the difference was statistically significant (P0.05).Three months and 6 months after wound healing, VSS scores for scar extent in T-group were 7.00±1.21 and 4.25±0.97, lower than that in C-group at the same time points, and the difference was statistically significant (P0.05). Morbidity rate of scar contracture in T-group was 33.33%, lower than in C-group, and difference was statistically significant (P0.05). Conclusion CS-SGM-D could promote repairing and dermal regeneration of deep burn wound, with good clinical effect.
