摘 要:目的探讨痰和血浆γ-干扰素(IFN-γ)含量检测对恶性血液病并发肺结核诊断的临床意义。方法采用酶联免疫吸附剂测定法分别检测并比较33例恶性血液病并发肺结核患者(恶性血液病肺结核组)、34例无恶性血液病的肺结核患者(非血液病肺结核组)、36例恶性血液病无肺结核患者(恶性血液病对照组)、36例健康工作人员(健康对照组)痰和血浆中IFN-γ的含量。结果非血液病肺结核组痰IFN-γ含量高于血浆中含量,且痰IFN-γ含量低于健康对照组,血浆IFN-γ含量高于健康对照组;恶性血液病肺结核组痰IFN-γ含量低于恶性血液病对照组,血浆IFN-γ含量高于恶性血液病对照组,差异均有统计学意义(P<0.05)。恶性血液病肺结核组和非血液病肺结核组痰、血浆IFN-γ含量比较差异均无统计学意义(P>0.05)。结论检测痰和血浆IFN-γ含量可以作为恶性血液病并发肺结核诊断(尤其是菌阴肺结核)的重要辅助指标。Objective To investigate the diagnostic significance of IFN-γ content detection in sputum and serum of the patients with maligant hematologic disease complicated with Pulmonary tuberculosis. Methods 33 patients with hematologic malignancies complicated with pulmonary tuberculosis were chosen as the hematologic malignancies and pulmonary tuberculosis group;34 pulmonary tuberculosis patients without hematologic malignancies were enrolled as tuberculosis group; 36 patients with hematologic malignancies but no pulmonary tuberculosis were chosen as hematologic disease group. In addition ,36 health people were chosed as the health control group. The level of IFN-γ in sputum and serum were detected by enzyme-linked im- munosorbent assay. Results The level of IFN-γ in sputum of the tuberculosis group was higher than that in the plasma, but which was lower than that of the health control group, and the IFN-γ level in plasma of the tuberculosis group was higher than that of the health control group; In hematologic malignancies and pulmonary tuberculosis group, the level of IFN-γ in sputum was lower than that of the hematologic disease group, however, the level of IFN-γ in plasma was higher than that of the hemato- logic disease group. The differences were statistically significant ( P 〈 0.05 ). No significant difference was found as regards the level of IFN-γ,in sputum and serum between hematologic malignancies and pulmonary tuberculosis group and the tuberculosis group ( P 〉 0.05 ). Conclusion IFN-γ, in sputum and/or serum is an important auxiliary indicator in the diagnosis of the he- matologic malignancies complicated with pulmonary tuberculosis,especially the bacterial culture negative pulmonary tuberculosis.
