烧伤休克改进公式补液牙龈血液灌流量改变的研究

摘  要:目的观察家兔烧伤后不同补液方案下早期牙龈血液灌流量(GH)动态变化,探讨改进烧伤补液公式的效果。方法 24只家兔分为对照组和实验组,每组12只,先复制烧伤动物模型,依据国内常用公式即:补液量(ml)=体表面积(total body surface ariea,TBSA)%×体重(kg)×1.5+生理需要量,进行补液的为对照组,依据改进公式即:补液量(ml)=TBSA(%)×体重(kg)×2+生理需要量,补液的为实验组。再应用LISCA激光多普勒组织灌流图像仪(LDPI),连续观测家兔烧伤补液前后8h内牙龈血液灌流量的改变,数据采用重复测量设计的方差分析。结果对照组烧伤前GH值(1.32±0.30)V,烧伤即刻,GH为(1.78±0.38)V(P<0.05),烧伤后8.0h,GH值逐渐减小。实验组灌流值即刻增大后,呈下降的趋势,但与对照组相比,GH值变化较缓。牙龈血液灌流图象的色彩也逐渐变淡。结论烧伤休克期应用改进公式补液,早期牙龈GH增加。Objective To observe the dynamic changes of gingival hemoperfusion(GH)according to the modified formula for fluid replacement in rabbits with burn shock.Methods Twenty-four adult rabbits were randomly divided into control group and experiment group,with 12 rabbits in each group.Firstly the animal models with burn were established,then the rabbits in control group were treated with routine formula of fluid replacement,i.e.fluid infusion amount (ml)=total body surface area (TBSA)%×body weight (kg)×1.5+physiological needs amount,however,the rabbits in experiment group were treated by the modified formula that was fluid infusion amount (ml)=TBSA (%)×body weight (kg)×2+physiological needs amount.Lastly the changes of GH at 8 hours before and after burn were detected continuously by LISCA Laser Doppler Hemoperfusion image instrument.The data were treated by repeated measure contrivable analysis of variance with SPSS (13.0) statistics software.Results Before burn the GH value in control group was 1.32±0.30V,which was 1.78±0.38V on instant moment of burn (P0.05),8 hours after the burn GH value was decreased gradually.The GH value in experiment group was increased on instant moment of burn,then which presented the tendency of decrease,however,as compared with that in control group,the GH value changed slowly,and the image color of gingival hemoperfusion was also thinned gradually.Conclusion When fluid replacement is carried out during burn shock by using modified formula,the gingival hemoperfusion value is increased at early period.
