球囊扩张后凸成形术治疗骨质疏松性椎体压缩骨折的疗效及安全性评价

摘　　要：目的:探讨应用球囊扩张后凸成形术治疗骨质疏松性椎体压缩骨折的疗效及安全性,寻找提高疗效、降低并发症的有效方法。方法:将骨质疏松性椎体压缩骨折患者进行分组,应用球囊扩张后凸成形术治疗组(BKP组,n=26),因本人意愿或一般状态较差采取保守治疗采取对症、卧床、制动等非手术治疗组(NSM组,n=25),2组患者年龄、性别、疼痛程度、骨质疏松程度、骨折类型及程度方面无差异,2组患者在术后6个月进行随访,比较二者VAS评分、椎体楔变角度变化及术后并发症。结果:BKP组患者术后6hVAS评分为1.6分,术后6个月VAS评分为2.7分,术后楔变Cobb角度11.6°,术后6月楔变Cobb角度为16.3°。NSM组患者保守治疗6个月后VAS评分为5.7分,6个月楔变Cobb角度为22.4°。BKP组患者术后即时及6个月VAS评分较NSM组明显降低,楔变Cobb角明显改善,组间比较差异有统计学意义(P<0.05)。2组患者均未出现严重并发症。结论:应用球囊扩张后凸成形术治疗骨质疏松性椎体压缩骨折疗效确切,安全性较高。Objective To explore the efficacy and security of ballon kyphoplasty（BKP）in treatment for osteoporosis vertebral compression fractures,and to search for effective mthods to improve curative result and decrease complication.Methods 51 patients were divided into BKP group and NSM group by patient＇s will or general conditon,26 patients were treated with ballon kyphoplasty（BKP group）,25 patients were treated with bed rest,analgesics and bracing（NSM group）.There were no differences in age,gender,VAS,BMD and severity of fracture between two groups.15 and 18 patients were followed up for 6 months respectively in BKP and NSM groups,the VAS,vertebrae wedge angle and complications were observed and compared.Results In BKP group,the VAS were 1.6 and 2.7 in 6 h after operation and 6 months after operation,respectively.The Cobb angle were 11.6°and 16.3° in 6 h after operation and 6 months after operation,respectively.In NSM group,the VAS were 5.7 and the Cobb angle were 22.4° in 6 months after operation.Compared with NSM group,BKP group get better VAS,kyphosis reduction（P0.05）,without serious complications.Conclusion The treatment of BKP for osteoporosis vertebral compression fractures is effective and reliable.
