磁共振弥散加权成像监测急性脑静脉性血栓形成的实验研究

摘  要:目的探索磁共振弥散加权成像监测大鼠急性脑静脉血栓形成(CVT)发生、发展的意义。方法建立急性脑静脉性血栓形成的大鼠模型,分别于0.5,1,3,6,12,24,48 h行MR常规T1加权成像、T2加权成像及弥散加权成像,测量病灶的大小,与切片的TTC染色结果进行比较并测定脑含水量。结果与T2WI比较,CVT在0.5,1,3 h,DWI显示的病变敏感(P<0.05)。而在6,12,48 h显示的病变范围两者间均无统计学差异(P>0.05);脑静脉性血栓形成0.5 h后缺血区ADC值逐渐下降,约为正常脑组织的(58.5±3.0)%,在此之后表观弥散系数(ADC)值逐渐增加,于48 h达到正常脑组织的(83.1±4.6)%;TTC染色结果与DWI病变位置和范围基本一致;CVT在上矢状窦(SSS)闭塞后6 h脑含水量即显著升高,为0.789±0.004,24 h最为明显,为0.799±0.002。结论脑静脉血栓形成病变发生、发展的时间过程及表现可通过磁共振弥散加权成像(DWI)加以监测,常规的自旋回波成像结合弥散加权成像可能对急性脑静脉血栓形成的早期诊断、预后评价和指导治疗有意义。Objective To explore the significance of monitoring acute cerebral venous thrombosis occurrence and development by diffusion weight imaging. Methods Rat model of acute cerebral venous thrombosis was established, Conventional MR Tl-weighted images,T2 weighted images and diffusion weighted imaging were performed continuously at different time points 0.5,1,3,6,12,24,48 h. The size of the lesions was measured and the slice TFC dyeing results were compared. Brain water content was determined. Results At 0.5,1,3 h time point,lesions discovered by CVT were more sensitive than T2WI . At 6,12,24,48 h time point,there was no difference of lesion area be- tween DWI and T2WI (P 〉 0.05 ). ADC values gradually decreased to 58.5%-+3.0% of normal level at the 0.5 h time point and then in- creased until the 48h observation point (83.1%±4.6% of normal). TIC dyeing results and DWI lesions position and scope is consistent. Af- ter the SSS was blocked for 6 h, CVT was a significant rise in moisture content, which was most obvious at 24 h. Conclusion MR diffusion weighted imaging can be used to monitor occurrence and development of experimental cerebral venous thrombosis ; the conventional SE imag- ing with diffusion weighted imaging could be significant in the early diagnosis, prognosis and guiding treatment of acute cerebral venous thrombosis.
