腹膜透析患者肾性贫血治疗的经济负担研究

摘　　要：目的:本研究测算腹膜透析患者采用利血宝(EPO)治疗肾性贫血的经济负担,包括直接费用(直接医疗费用与直接非医疗费用)和间接费用。方法:通过在8个三级医院透析中心的前瞻性观察研究,随访记录患者的基本情况以及3个月内疾病治疗情况与经济负担情况。结果:完成112例患者随访,合计485人次门诊就诊和15人次住院。患者平均年龄为51.72岁,平均透析年数3.87年,EPO平均使用3.69年;患者平均每月在临床中心门诊就诊1.24次,在社区就诊1.01次;每月EPO的平均费用为1 379元,EPO用量为13 645 IU;患者每月人均经济负担为9 658元,其中,直接医疗费用9 061元,直接非医疗费用206元,间接成本390元。结论:腹膜透析加肾性贫血患者的经济负担较重。Objective： This study aims to estimate the total economic burden of both peritoneal dialysis and renal anemia treatment in China. Methods： 8 medical centers with regular follow-up system for peritoneal dialysis patients were selected tbr prospective observation research. During 3-months follow-up duration, direct medical eosts, direct non-medical costs and indiret costs were collected. Results： 112 patients with records of 485 outpatient visits and 15 inpatient stays were followed up. The mean age of patients is .51.72 with averagely 3.87 years of peritoneal dialysis treatment and 3.69 years of EPO treatment. The average number of visits per month was 1.24 visits in medical centers and 1.01 visits in community health centers. The average cost of EPO was 1 379 yuan per month with average dosage of 13 645 IU. Total economic burden per month per peritoneal dialysis patient was 9 658 yuan. including 9 061 yuan for direct medical eosts, 206 yuan for direct non-medical costs and 390 yuau for indirect costs. Conclusions： The total economic burden of both peritoneal dialysis and renal anemia treatment was relatively high, which needs more attention from the government and the society.
