血液回收与回输技术在急诊大出血手术中的应用

摘　　要：目的探讨自体血液回收与回输技术在急诊大出血手术中临床应用的效果。方法选择本院2011年1月～2012年5月急诊大出血的患者中,采用国产ZITI-3000型血液回收机(北京京精医疗设备公司生产)行血液回收与回输的患者30例,其中回收血量超过800mL的患者进行血液清洗,同时监测患者入室时、血液回输前及血液回输后、出室时的SBP、DBP、MBP、HR、SPO2、RBC、Hb、HCT、PT、APTT值。结果每位患者回收血量平均为1910mL,总回输血量平均为678mL,所有行血液回收的病例中,除1例患者因下腔静脉破裂血管缝合困难造成短时间大量出血发生失血性休克死亡外,其他患者血液经洗涤回输后生命体征均有明显改善,术后血红蛋白较入室时明显提高。所有抢救成功的患者均未发生多脏器功能障碍,且未见输血相关并发症。结论 ZITI-3000型血液回收仪可安全有效的应用于急诊大出血手术的患者,并且显著减少术中血液的丢失,以及降低输血相关并发症发生率。Objective To observe the effect of blood recovery and retrieval technology by using ZITI-3000 Autologous Blood Recovery System in emergency bleeding surgery. Methods Thirty patients undergoing emergency bleeding surgery was enrolled using ZITI-3000 Autologous Blood Recovery System from January 2011 to May 2012, the blood was cleaned for the recovery of blood more than 800 mL, the SBP, DBP, MBP, HR, SPO2, RBC, Hb and HCT was dectected during the patients enter the operation room, before blood transfusion, after blood transfusion and the patients leave the operation room. Results Blood recovery average of each patient was 1 910 mL, the average total return of the amount of blood trans- fusion was 678 mL, exception of one patient was died in hemorrhagic shock due to the inferior vena cava rupture but diffi- culties to suture in short time, the other patients＇ vital signs and hemoglobin were improved significantly. All patients with successful treatment did not occur with multiple organ dysfunction and transfusion-related complications. Conclusion ZITI-3000 Autologous Blood Recovery System is very useful and efficacious for decrease loss of blood during operation and complications of homologus blood trasfusion.
