摘  要:目的探讨静注免疫球蛋白(IVIG)与大剂量地塞米松(DEX)对特发性血小板减少性紫癜(ITP)患儿免疫功能的影响。方法将65例ITP患儿随机分为两组,IVIG治疗组35例,DEX治疗组30例;两组治疗前后各抽血一次,分别测定患儿的免疫球蛋白及血小板计数。结果 (1)IVIG组治疗后,IgG显著升高,与治疗前相比,差异具有高度统计学意义(P<0.01);DEX组治疗后,IgA、IgG、IgM均降低,与治疗前相比,差异具有统计学意义(P<0.05)。(2)治疗后,IVIG组与DEX组相比,血小板数量差异无统计学意义(P>0.05)。结论 ITP患儿DEX治疗组体液免疫功能下降,建议对ITP患儿应尽量选用IVIG治疗。Objective To discuss the influence on intravenous Immunoglobulin (IVIG) and high-dose Dexamethasone (DEX) action on the immune function of children with idiopathic thrombocytopenic purpura (ITP). Methods Sixty-five children with ITP were randomly divided into two groups, IVIG therapy group had 35 cases, DEX treatment group had 30 cases; Two groups were given haemospasia before and after treatment, immunoglobulin and platelet count of children were measured. Results (1)After treatment, lgG of IVIG group was significantly increased which was ompared with the pre- treatment had significant difference (P 〈 0.01); IgA, IgG and IgM of DEX group were decreased after treatment, which compared with before treatment had significant difference (P 〈 0.05). (2)IVIG Group compared with DEX group after treat- ment, there was no significant difference on the platelet count (P 〉 0.05). Conclusion Humoral immune function of chil- dren in the DEX group decline, so IVIG therapy should be used in the treatment of children with ITP.
