凉山州彝族、汉族多囊卵巢综合征患者的特点及治疗差异性

摘　　要：目的探讨凉山州彝族、汉族多囊卵巢综合征(PCOS)患者内分泌代谢紊乱的特点及治疗差异性。方法选择2009年1月至2011年12月在本院妇科门诊就诊的PCOS患者80例为研究对象,其年龄为20～39岁,按照不同民族,将其分为彝族组(n=40)和汉族组(n=40)(本研究遵循的程序符合本院人体试验委员会制定的伦理学标准,得到该委员会批准,分组征得受试对象的知情同意,并与其签署临床研究知情同意书)。两组患者的年龄、不孕年限等比较,差异无统计学意义(P>0.05)。对两组患者给予炔雌醇环丙孕酮(达英-35)+二甲双胍治疗3个月,观察治疗前、后性激素及糖代谢指标、体重指数(BMI)、卵泡变化情况等。结果彝族组及汉族组黄体生成素(LH),睾酮(T),卵泡刺激素(FSH)及LH/FSH水平均较治疗前明显下降(P<0.05),治疗后两组葡萄糖和胰岛素的曲线下面积(AUC)均明显下降(P<0.05)。结论炔雌醇环丙孕酮+二甲双胍对降低PCOS患者内分泌代谢紊乱疗效明显,但彝、汉族之间临床表现及疗效有一定差异。Objective To discuss treatment differences and clinical characteristics between Yi and Han nationality in Liangshan Yi Autonomous Prefecture with polycystic ovarian syndrome （ PCOS ） . Methods From January 2009to December 2011,a total of 80cases with PCOS were included into this study.They were divided into Yi group （ n=40 ） and Han group （ n=40 ） according to their nationalities. The study protocol was approved by the Ethical Review Board of Investigation in Human Being of First People′s Hospital of Liangshan Prefecture.Informed consent was obtained from each participate.There had no significant differences between two groups among age,infertility years,and so on （ P0.05 ） .All the subjects received dimethyl biguanide＋ metformin hydrochloride for 3 months,and the glycometabolism indexes,body mass index （ BMI ）, and egg chamber changes were observed before and after the treatment. Results The level of LH,T and LH / FSH ratio decreased significantly than those before treatment （ P 0.05 ） .After treatment,the glucose and insulin area under the curve （ AUC ） were significantly decreased （ P0.05 ） .Conclusions Dimethyl biguanide combined with metformin can reduce metabolic disorders of PCOS.But obvious differences between Yi and Han were found.
