摘  要:采用文献综述法,对缺血修饰白蛋白、心型脂肪酸结合蛋白、肌酸激酶同工酶、心肌肌钙蛋白、B型钠尿肽和C反应蛋白等心脏分子标志物的生物学特征进行概述,探讨运动引起心脏分子标志物的变化及其可能机制,分析心脏分子标志物用于评价运动性心肌损伤的特点和意义。研究表明:理想的评价运动性心肌损伤的心脏分子标志物,主要应具备高度的心肌特异性和敏感性,其释放比例与心肌损伤程度一致,并有较长的半衰期;心肌肌钙蛋白因具备较高的心肌特异性和敏感性等特点,是评价运动性心肌损伤较理想的心脏分子标志物;在评价运动性心肌损伤时,可首选心肌肌钙蛋白或联合应用其他心脏分子标志物,有助于综合评价运动性心肌损伤的程度。This paper uses the method of literature review to summarize the biological characteristics of the cardiac biomarkers, such as ischemia-modified albumin, heart-type fatty acid-binding protein, creatine kinase-M, cardiac troponin, B-type natriuretic peptide and C-reactive protein. The paper discusses the influence of exercise on cardiac biomarkers and its possible mechanism; analyze the features and significance of cardiac biomarkers for evaluating exercise-induced myocardial injury. These studies indicate that an ideal cardiac biomarker for evaluating exercise-induced myocardial injury should have a high cardiac specificity and sensitivity, the release of which should be consistent with the degree of myocardial injury and also has a long half-life. Cardiac troponin, which has a high cardiac specificity and sensitivity, can act as an ideal cardiac hiomarker for evaluating exercise-induced myocardial injury. When evaluating exercise-induced myocardial injury, cardiac troponin can be selected as the first choice, or other cardiac biomarkers are combined in order to comprehensively evaluate the degree of exercise-induced myocardial injury.
