摘　　要：目的研究老年勺型和非勺型高血压对心脏功能的影响。方法选取2010年1月至2011年12月我院78例Ⅰ～Ⅱ期原发性高血压患者,进行动态血压监测,将动态图形分为勺型组与非勺型组,各39例,同时选取51例健康同龄人群对比。用动态血压与心脏超声检测3组患者的各项数据,进行统计学分析。结果非勺型高血压组的左心室结构与功能与对照组比较,均发生了显著的变化(P<0.01),而勺型高血压组的LVDd变化与对照组比较无显著变化;非勺型高血压组的24h SBP、dSBP、nSBP、SBP负荷值和勺型高血压组以及对照组比较均显著升高(P<0.01),而24h DBP、dDBP以及DBP负荷值差异无统计学意义。结论非勺型高血压比勺型高血压患者更加容易发生舒张功能障碍,值得临床关注。Objective To study the effect of the elderly dipper and non-dipper hypertension on the left ventricular function. Methods Seventy-eight patients with StageⅠ～Ⅱ essential hypertension were divided into the dipper group and the non-dipper group, with 39 cases in each group. Fifty-one healthy individuals were selected as the control group. Ambulatory blood pressure and ultrasound echocardiography were employed to detect the relevant parameters of the three groups for statistical analysis. Results The structure and function of left ventricle in the non-dipper group showed significant changes compared with the control group （P0.01）. The changes of LVDd in the dipper group showed no statistically significant difference with that in the control group. 24 h SBP, dSBP, nSBP, SBP in non-dipper group were significantly higher than those in the dipper group and the control group （P0.01）, while 24 h DBP, dDBP, DBP showed no statistically significant difference. Conclusion Non-dipper hypertension are more prone to diastolic functional abnormalities than dipper hypertension.
