影响新生儿抗生素相关性腹泻因素临床分析

摘　　要：目的探讨引起新生儿抗生素相关性腹泻(AAD)相关因素及预防措施。方法选择应用抗菌药物的1450例新生儿作为研究对象,将AAD患儿作为观察组,按照1∶1比例选择无AAD患儿作为对照组,比较两组影响AAD因素差异。结果 1450例患儿中发生AAD110例,观察组患儿胎龄(35.23±2.32)周、年龄(9.13±3.56)d、纯母乳喂养12.72%,低于对照组的(36.97±2.04)周、(21.78±4.64)d、31.82%;广谱抗菌药物应用71.82%、抗菌药物(2.11±0.17)种、预防应用抗菌药物32.73%、给药时间(10.11±2.16)d、实施侵入性操作(33.64%),高于对照组的54.55%、(1.72±0.12)种、10.00%、(7.45±2.34)d、13.64%(P<0.05),是发生AAD的相关因素。结论 AAD的发生率较高,与多种因素有关,通过加强围生期保健,减低早产儿的发生;掌握使用抗菌药物的指征;提倡母乳喂养;减少或者避免侵入性操作进行预防。OBJECTIVE To explore the causes of antibiotics-associated diarrhea（AAD） in neonates and prevention countermeasures.METHODS A total of 1450 neonates who were treated with antibiotics were selected as the study objects,and the neonates without AAD were chosen,in a 1∶1 ratio,as the control group,the factors that may affect AAD were analyzed.RESULTS Of 1450 cases of neonates,AAD occurred in 110 cases,the children of the observation group gestational age（35.23±2.32） weeks,age（9.13±3.56）days,pure breastfeeding was 12.72%,less than 31.82% of the control group（36.97±2.04） weeks,（21.78±4.64） days;the use of broad-spectrum antibiotics was 71.82%,antibiotics（2.11±0.17）types,and prophylactic use of antibiotics 32.73%,medication time（10.11±2.16）days,implementation of invasive operation 33.64% in the observation group,higher than 54.55%,（1.72±0.12） kinds,10.00%,（7.45±2.34） days,and 13.64% in the control group（P0.05）,which were the related factors for AAD.CONCLUSION The incidence of AAD is high,which may be related to multiple factors so that it is necessary to strengthen the healthcare during perinatal period so as to reduce the premature newborns;it is also important to grasp the indications of use of antibiotics,encourage the breastfeeding,and reduce or avoid the invasive operation so as to prevent AAD.
