循证在消毒供应室集中式管理模式转变中的应用

摘　　要：目的探讨循证护理管理在消毒供应室集中式管理模式转变中的应用。方法分析影响集中式管理模式转变的因素,确立循证问题,严格评价文献,将真实可靠的证据与管理知识经验及医院实际情况相结合,做出管理决策,并通过实践评价,循环往复提高管理决策和管理过程的科学性、实用性及持续改进。结果在6个月的时间内实现了管理模式的转变,实施集中式管理后,清洗质量抽样合格率达99.1%,包装质量抽样合格率达100.0%,灭菌质量抽样合格率达100.0%,临床科室满意率达98.6%,工作人员在器械再生过程中锐器伤发生率为0;平均器材灭菌总量11 505L/d,每件器械周转利用次数平均1.5次/d;用人成本0.23万元/(月.人),设备维修保养成本约1万元/年。结论应用循证护理管理方法,以科学的研究依据来完成管理决策和管理过程,以更科学、更可行的新思路及新方法解决实际问题,实现消毒供应室集中式管理模式的快速平稳转变,高质量、高效率、低成本地实行重复使用医疗器械的集中供应。OBJECTIVE To discuss the application of the evidence-based nursing management in the transferring of the centralized management mode in central sterile supply department.METHODS The related factors for the transferring of the centralized management mode were analyzed,a position was developed on the issue of evidence-based nursing,the judgments were made on the documentations strictly,the management decision was made by the combination with the real and reliable evidence and the manage experience and the practical situation of the hospital,the evaluation was made by the practice,the management decision and process were improved in circle,and the scientific,practical and sustainable management process was improved as well.RESULTS The transferring of the management mode was successfully achieved within 6 months,the qualified rate of the sampling of cleaning quality was 91.1%,the qualified rate of the samples＇ packing quality was 100.0%,the qualified rate of sampling of sterilization quality was 100.0%,the satisfaction rate of the clinical departments reached up to 98.6%,the incidence rate of sharp instrument injuries during equipment regeneration process was 0%;the average of total number of the sterilized equipments was 11 505 L/d,the average utilization rate of the equipments was 1.5 times/day,the cost of the employees was 2 300 yuan/month·person,the cost of equipment maintenance was almost 10 000 yuan/year.CONCLUSION The evidence-based nursing management method,as involves in the management and process based on the scientific reference and solve the problems in a more scientific and feasible thinking approach,can help to realize a rapid and smooth transferring of centralized management mode in CSSD and make the centralized supply more qualified,efficient and low-cost.
