摘  要:目的分析引起深部真菌感染的假丝酵母菌属在临床标本的分布及耐药性,以更好地指导临床预防及治疗抗真菌感染,提高疗效。方法用科玛嘉显色培养基和法国生物梅里埃公司ATB的ID 32C做菌种鉴定,ATBFunguns3测定氟康唑、5-氟胞嘧啶、两性霉素B、伊曲康唑、伏立康唑的敏感性,并对假丝酵母菌属的鉴定结果与耐药情况进行分析。结果 6722株标本中分离出的假丝酵母菌属878株,检出率为13.06%,对有临床意义的假丝酵母菌属进行药敏试验和分析,其中白色假丝酵母菌、光滑假丝酵母菌、近平滑假丝酵母菌、热带假丝酵母菌、克柔假丝酵母菌、葡萄牙假丝酵母菌,分别占32.02%、20.69%、18.72%、14.29%、9.58%、2.95%;不同假丝酵母菌对抗真菌药物的敏感性存在差异,白色假丝酵母菌对氟康唑耐药率为43.08%,5-氟胞嘧啶为40.00%,两性霉素B为32.31%,伊曲康唑为29.23%,伏立康唑为32.31%。结论目前治疗临床深部真菌感染的药物,氟康唑、5-氟胞嘧啶、两性霉素B、伊曲康唑、伏立康唑有较好的抗菌活性,加强假丝酵母菌属耐药性的检测,为临床提供合理用药依据,减少耐药性的增加。OBJECTIVE To analyze the distribution and drug resistance of Candida causing deep fungal infections isolated from clinical specimens in order to better guide the clinical prevention and treatment of fungal infections and raise the treatment efficacy.METHODS Candida isolates were identified by Kema Jia Chromogenic Medium and ATB-ID 32C,fluconazole,5-Flucytosine,Amphotericin B,Itraconazole and Voriconazole antifungal susceptibility were tested by ATB Funguns 3,the result of the identification of Candida and the drug resistance status were analyzed.RESULTS Of 6722 clinical specimens,there were 878 strains of Candida were isolated with the detection rate of 13.06%,we performed the drug susceptibility testing for the clinically significant Candida strains including C.albicans(32.02%),C.glatraba(20.69%),C.parapsilosis(18.72%),C.tropicalis(14.29%),C.crusei(9.85%),and C.lusitaniae(2.95%);the drug susceptibility testing showed that Candida varied in the drug susceptibility to antifungal agents,the drug resistant rate of C.albicans to fluconazole was 43.08%,5-Flucytosine 40.00%,amphotericin B 32.31%,itraconazole 29.23%,voriconazole 32.31%.CONCLUSION At present,the antifungal agents for the deep fungal infections such as fluconazole,5-Flucytosine,amphotericin B,itraconazole and voriconazole have significant antibacterial activity,so it is necessary to strengthen the detection of drug resistance of Candida so as to provide basis for the reasonable use of antibiotics and reduce the drug resistance.
