化纤破痼汤治疗特发性肺纤维化的研究

摘　　要：目的:研究化纤破痼汤联合西医常规疗法对特发性肺纤维化(IPF)的临床疗效及其对肺功能、血气和TGF-β1的影响。方法:50例IPF患者随机分为两组,西医对照组25例按照西医常规疗法治疗;中医组25例在西医对照组基础上口服化纤破痼汤(药用:黄芪、当归、生地、淫羊藿、连翘、桔梗、枳壳、杏仁、五味子、白芍、法半夏、制南星、川贝、陈皮、川芎、桃仁、地龙、茯苓、生甘草等),疗程90天。观察临床疗效,测定治疗前后VC、TLC、DLCO、PaO2、血中TGF-β1浓度。结果:中医组总有效率92%,高于西医对照组(68%)(P<0.05);治疗后中医组VC、TLC、DLCO、PaO2均较治疗前显著升高(P<0.001),与西医对照组治疗后比较有显著性差异(P<0.01,0.05);中医组治疗后血中TGF-β1浓度较治疗前明显下降(P<0.05),西医对照组则无显著性差异。结论:化纤破痼汤联合西医常规疗法能更有效提高IPF临床疗效,改善肺功能和缺氧,化纤破痼汤具有一定抗肺纤维化作用。Objective：This study was aimed at observing the curative effect of Huaxian Pogu Formula on patients with idiopathic pulmonary fibrosis（IPF）.Methods：50 patients with IPF were divided into two groups at random.The Traditional Chinese Medicine（TCM）group（25 cases）was treated with Huaxian Pogu Formula and the Western medicine convention treatment,the Western medicine matched group（25 cases）was treated by the Western medicine convention treatment only.The course of the treatment was 90 days in both groups.50 cases in both groups were observed about the clinical manifestations and examined VC,TLC,DLCO,PaO2 and transforming growth factor-β1（TGF-β1）before and after therapy respectively.Results：The total effective rate in the TCM group（92%）was higher than that in the Western medicine matched group（68%）（P〈0.05）.VC,TLC,DLCO and PaO2 in the TCM group after therapy were higher than those before treatment and those in the Western medicine matched group after therapy（P〈0.05）.Significant decreases of the density of TGF-β1 were found in the TCM group after therapy（P〈0.05）.Conclusion：The combination therapy of Huaxian Pogu Formula and the Western medicine convention treatment can improve the clinic manifestations,pulmonary function and hypoxia in patients with IPF.Huaxian Pogu Formula has the anti-fibrosis function.
