参附注射液对急性心肌梗死合并心衰的治疗研究

摘　　要：目的:观察参附注射液对急性心肌梗死合并心衰患者的临床治疗效果,探讨参附注射液治疗急性心肌梗死合并心衰的作用机制。方法:78例急性心肌梗死合并心衰患者随机分为常规治疗组和参附组。常规治疗组给予常规抗心力衰竭治疗。参附组在常规抗心力衰竭治疗基础上加用参附注射液静脉输注。治疗前后进行心功能评价,并测定血浆脑钠肽(BNP)、超敏CRP(hs-CRP)、内皮舒张因子一氧化氮(NO)、肿瘤坏死因子-α(TNF-α)的水平。两组均有患者行急诊PCI治疗。结果:与治疗前相比,两组患者经治疗心功能均有改善,BNP、hs-CRP和TNF-α等的血浆水平均有下降,NO水平升高。与治疗组相比,参附组的各项参数改善更明显(P<0.05)。结论:参附注射液可改善急性心肌梗死合并心衰患者的临床症状,改善左心收缩功能。Objective：To observe the efficacy and its mechanism of Shenfu Injection on acute mocardial infarction combined with heart failure.Methods：78 cases of acute myocardial infarction combined with heart failure were randomly divided into routine treatment group and Shenfu group.Conventional treatment group was given conventional treatment of heart failure.Shenfu group was given routine treatment combined with intravenous infusion of Shenfu injection.Assessment of cardiac function before and after treatment and the measurement of plasma brain natriuretic peptide（BNP）,high sensitivity CRP（hs-CRP）,endothelial relaxing factor nitric oxide（NO）,vascular contraction factor and tumor necrosis factor-α（TNF-α） levels was made.In the two groups,there were some patients undergoing emergency PCI treatment.Results：Compared with that before treatment,heart function of two groups was improved.BNP,hs-CRP and TNF-α plasma levels dropped and NO level was increased.Compared with the treatment group,Shenfu group parameters were improved significantly（P〈0.05）.Conclusion：Shenfu injection can improve clinical symptoms of patients with acute myocardial infarction complicated with heart failure and improve left ventricular systolic function.
